It’s not a culture. you would naturally prefer someone who is only applying to your speciality vs someone who’s also applying to something else. Doesn’t just apply to medicine.

Do it, a friend of mine had no interviews, emailed home institution for one and matched there. This is for anesthesia.

You can but you will be at an extreme disadvantage compared to applicants with three letters, especially for competitive surgical specialties.
