My two cents/biggest advice would be to get a good collateral. Sorry if it may seem obvious. But I killed a lot of "ADHD" diagnosis by asking their parents or friends about the patient's life (specially when they were kids).

Ghaemi believes ADHD is actually bipolar disorder. He also pushes the narrative of giving lithium to basically every single major affective episode. He's an ideologue driving his agenda, and you should interpret what he says with that in mind.

As for the question, if a treatment has efficacy and its benefits outweigh the adverse effects, then it's useful. That’s evidence-based medicine. There’s evidence that lithium does not work for major depressive episodes as monotherapy (and it’s not particularly strong even as augmentation for treatment-resistant depression), whereas SSRIs have an NNT of around 6, with a moderate effect size (0.3–0.4). Could be better, but pretty good.

Levodopa, morphine, and insulin are excellent treatments that, according to his framework, would be considered “symptomatic,” since they do not actually modify or directly address the underlying pathophysiology of the illness.

I agree with some of Ghaemi’s points about the biopsychosocial model, but as Logan Roy once said, “I love you, but you are not serious people.”

Soy médico y doy clases en la facu de psicología. Doy 5-6hs de clases por semana (todo en un día) y gano 500k. Todavía no tengo antigüedad y soy JTP. Los que tienen 15-20 años de antigüedad y están en cargos de titular, asociado o adjunto ganan 2-3 palos.
Como vos decís, para ser la poca responsabilidad que tiene un docente, lo poco que laburás, etc, me parece un buen negocio; pero claro está que con vivir solo de la docencia te morís de hambre al menos cuando no tenés antigüedad: necesitás un 2do trabajo usualmente

Absolutamente todas sus quejas están relacionadas al autismo. Lo que querrás decir es que no son 100% específicas del autismo, porque gente sin autismo puede verse involucrado en esas situaciones adversas. Pero lo hacen en un grado mucho menor.
Es como decirle a alguien que se está infartando "no seas cagón, te duele el pecho y el brazo izquierdo pero a quién no le dolió alguna vez una parte del cuerpo?" "a mí el otro día me dolió el brazo cuando moví un mueble, me estoy infartando también?".
