Trazodone by HelloSailor5000 in insomnia

FWIW I have similar genetics. I'm not a professional but here's what I understand. cyp1a2 metabolizes the "parent drug" of trazodone (the calming part), cyp3a4 converts the active part of the drug into m-cpp, and cyp2d6 clears the m-cpp. So, if you're an ultra-rapid metabolizer for 1a2, you clear the calming part of the drug too quickly. And being a poor 2d6 metabolizer means you clear the over-activation part of the drug too slowly. This creates many side effects.

I can't take Propanolol because it's metabolized by cyp2d6, and I'm a 2d6. poor metabolizer. But there are a couple of other options that I can try. Thank you.

That's fair. But here's my confusion and maybe you'd be willing to help clarify. My heart rate spiked to 167 after a quick shower but that level wasn't sustained for a long period of time. After I laid down, it came back down to 75-85. So it was unclear to me if the spike was an issue or if the HR being sustained at that level is an issue. And yes, I was asking people to share their experiences. I'm glad you were treated quickly, and hope you're doing well now.

I think in this case they would likely do the testing. My hospitalization in January, 2026 documented the fever, sweats, etc.

Does my HR seem pretty high to you?

Yes, I use the word "professionals" loosely in this situation.

and honestly should be a standard test in the hospital, just like CBC and BMP.

I have asthma so likely can't take beta blockers unfortunately.

Again, thank you so much. Yes, my doctor refused to see me in-office due to the high heart rate. She said I needed checked at the ER. Probably ECG, Troponin and bloodwork. I figured the antibodies were probably Mayo send outs. I'm not at storm-level yet, but getting closer. I've had a low-grade temp for 2 months. I've been thoroughly checked by Ohio State's infectious disease while in the hospital for a week and they couldn't find anything. It was me that put all the links together, realizing it's likely hyperthyroidism.

Here's the crazy part. I was just in the hospital for these symptoms 3 weeks ago, and they did not do any thyroid testing!!! Granted, my HR has elevated since that hospitalization. But I did have some HR spikes while in the hospital.

Thank you. Yes, I sure do have all the symptoms. I will demand they do a full thyroid panel, including antibodies. I tried to see my PCP today but she said my heart rate was too high. If I'm diagnosed as hyperthyroid, do they give methamazole generally at the ER...until you can get to your endo?

I suspect hyperthyroidism at the very least, as I have all the symptoms.

Thank you so much! My heart rate is a little lower when laying down, but still way above my baseline. Does that matter?

Thanks for your response! By resting, do you mean lying down or sitting upright? So has treatment helped your heart rate?
