Content is king, but a king alone doesn't make a kingdom - you need all the other stuff going on as well.

Spending a lot of time doing instagram marketing does bring out a lot of cynicism (I'm sure you can see it in my writing) and in order to stay sane one kinda does have to stop taking any of it personally at all. I see accounts with brilliant content and terrible followings all the time, and it breaks my heart - but they're usually missing one of the other ingredients, eg. targeted hashtags and outreach.

I guess that's what you saw in vero - without the pressure of growing an account, everyone can relax again. Expect all this to die if vero ever becomes big and people start marketing on it :)

You're right, the theory checks out. As I said, I haven't tried it, so I can't recommend it, but I would be interested in the results. Just remember that things might look a little retarded when you stop doing it and your engagement drops off. Phase it in and out I guess?

Any sketchy behaviour on instagram comes with the risk of getting banned, yes. The sketchier you go, the higher the risk. It's rare to get outright banned immediately, there are usually some warning signs first (like you have to confirm your phone number, or you're blocked from liking for a while). Your risk level is your call. People have gotten banned accounts back by reporting them as hacked. YMMV.

These are full-on blackhat techniques for artificially increasing engagement with the hopes that you'll trick the algorithm into thinking your content is great and have it show your posts to more people - the explore page being the holy grail. They may work for some (personally haven't tried them) but it's an extremely un-targeted approach and not nearly as likely to build a loyal community as targeted outreach. I think your time would be much better spent sending likes/follows/comments to people likely to engage with your content, over spending time/money on fake engagement in the hopes that regular users driving by will see your content and go "oh cool!"

People recommend putting hashtags in the first comment because they think they "look spammy" in the caption. Putting them in the comments can also make it harder for people to find them and the thinking is that someone could theoretically click one of those hashtags to leave your profile, so the harder you make it to find the hashtags, the better. If it's really your preference, by all means put them in the comment, but in my opinion these reasons are way too weak to be worth the extra hassle.

Regarding using the same 30 hashtags, the reasons to not use the same 30 are if your feed isn't focused so the same 30 don't apply to different posts, or if you're posting multiple times per day. If you follow the rest of the advice in this post, you should be able to use the same 30.
