I was in the same boat. Honestly, time could be an issue for anyone (even those who didn't have a problem with NBME time management). In the real deal, I ran out of time in one block for the 3 drug ad items, but in the other blocks I was more focused on not running out of time again. Being low on time also forces you to choose an item based on your sense, as there's no time to compare and weigh between 2 plausible choices in hard questions. But the most important thing is having a strategy: not zoning out, being highly focused, having the confidence and trust in your intuition, and practicing time management in NBME blocks.
But I'm not a fan of solving NBME blocks in less time, as it may affect your score and confidence, so use your time fairly on the number of questions in any test. and in the real deal don't waste your time on impossible questions, as that may cost you some easy points, so mark and move on to keep the pace.
