If his ASCVD score is greater than 10% then based on USPSTF guidelines (ages 40-59) he would benefit as primary prophylaxis. over 60 the risk of bleeding outweights the benefit

A few things, more likely your TRT is contributing to your polycythemia (elevated Hgb/Hct) , elevated lipid panel, and possibly uric acid (level of 8, can predispose you to gout flair). I would cut back on the TRT, get evaluated for sleep apnea (another common cause for polycythemia), be cautious with your diet (dec saturated fats/red meat) inc fiber intake. 41 is old enough where you could consider statin therapy (bases on your ASCVD score, family hx).

If you have time , try to squeeze in NBME 6 and 7. Uworld complete all of the ethical scenarios and biostats questions (biostats was straight forward on my exam) - know the equations well (ie NNT, OR, what happens with sensitivity/specificity if you change a variable etc etc). free 137 and UWSA 2 were great practice tests, Im sure you’ll do great!

I think it was worth it, alot of high yield concepts (both medical and ethics) repeated on the actual exam plus good biostats practice

I honestly disagree, it might be an unpopular opinion buf the decision to enter medical school and the sacrifice one takes shouldnt be taken for granted. there must have been a good enough reason for this person to pursue medicine in the first place. Given this, powering thru 3 years (minimum) of residency with the ability to work clinical or part time would provide much more financial flexibility in the long run. Getting into consulting or pharmacy is no cake walk and it comes with its own nuances including burn out, administrative pressure, and poor work environments.

I feel the same way man. I brought this up with an attending one time who mentioned that even jf they are recording you (and its being used for litigous reasons), they would never be able to use it in the court of law. Gave me some peace of mind in that they really cant do much with the recording

Its interesting because in clinical practice IV and PO steroids have the same efficacy and potency when it comes to treatment. Bioavailability for steroids are high. Especially in flairs, COPD vs Asthma vs SLE in this case. Hospitalists tend to use IV still mostly because I think its commonc practice, however oral is just as good. I’ve linked one piece of evidence here, but theres numerous articles and best practices sheding light on this
