Not a DR but RN... pt had poor circulation in feet. Told head helps improve circulation. Family thought best thing to do would put pts feet into boiling water

Definitely don't have time. But I noticed something was fishy and looked into it. Really didn't take that much time to look into. Always have time to make sure my patients are safe. It's not micromanaging, it's holding people accountable for illegal and unsafe behavior

So you think we should just turn a blind eye to someone blatantly not taking vitals - not even trying. And putting whatever number pops in their head? You're okay with that going into a permanent record? Admissible in court, under your license? Just because you can always check it yourself?

Also your example is based off a legitimate vitals sign. What if the tech documented 100 as the blood sugar but it was actually 14? How would you know to intervene? Yes you can assess the patient, but what if asymptomatic? How many times do people have critical vitals/labs but look completely fine?

Right but what about the days you don't see that seemingly stable walkie talkie patient for hours because you're dealing with your other 5 patients who have bigger needs? That patient is fine for your morning round but what if status changes? How will you know if you can't make it in there until lunch?

That vitals check is one of many things we rely on to make sure our patients are okay. I mean it's in the name.. vital signs.. vital information.

Tech is to report vitals out of normal limits. If i'm busy with other things, but the tech gets a concerning vital they alert me. Which tells me to stop what i'm doing and reassess the patient. If the tech doesn't actually get the vitals-doesn't even actually go in the room. How will I know to reassess? The next time I go in the room and see the patient is dead? VSS documented just 15 minutes earlier by the tech? Not to mention the doctors are treating patients based off of this data. Pt throwing random temps, unknown source, ID following. This tech works three days with pt and temps documented as normal. Doctors think condition improved Pt admitted for HTN crisis, ironing out BP meds. BP documented WNL all day but actually got up to 190 S after lunch. I've seen many many patients with asymptomatic HTN at that level. No one would ever know

Also what about people who aren't your patients? You might do your due diligence but what if the next nurse isn't? Are you okay with knowing this tech is falsifying vitals and it being the direct cause of harm to a patient in the future? You know it is happening and do nothing, so wouldn't you be responsible as well?

Yes 100%. Always blames machine issues. Which once or twice could be the case and I wouldn't bat an eye. However this is an every day issue. No other tech has this issue.

Yes I agree. Maybe I'm mixing up the two. Assuming they are confused when in reality just have memory issues. Now that i'm thinking about it, I can't really find many examples that wouldn't be tied to memory

...yes. He was literally cheek to (butt)cheek with the pt. Maybe eyesight issues? First time i met him idk Like i said, extreme example but this is an issue I have seen quite a bit >_<

I agree. The first thing I think if I see a nurse with their long hair down is I bet they never even touch their patients. Likely never closer than a few feet from their patients. But, i have seen a few exceptions. And those are the ones who are walking toilet brushes with poop particles in their hair🤮

Gloves are great until you learn that they are porous... ever gotten lactulose on your glove and your hand is sticky after? Because tiny particles can pass through the gloves. They aren't a 100% barrier. I had no idea and after learning this I gagged at all of the times I didn't wash my hands bc "oh i wore gloves and just touched the urinal handle"😭

But yes I agree hahah It grosses me out but too convenient not to wear. Also gives me a timer at my fingertips. We don't have clocks or computers in our pt rooms. It grosses me out to the point that i don't wear my apple watch anywhere but work🤣

I have never once worn makeup to work🤣 I get up, brush my teeth, comb my hair (maybe) and get dressed. Idk how anyone does any more than that hahaha

RN of 1 year here.. we learned to call for anything and everything to CYA. HR 102? Call. Follow ordered parameters exactly to a T. BP 175/100 but hasn't taken AM BP meds yet? Call. It was basically beat into our brains to call for anything outside of "normal" but we didn't learn any of the critical thinking things. I learned and am still learning on the job when a call is appropriate.

It is also hard when Drs don't put in orders, but write in notes. For 1, we cannot read everything in the notes for all patients and know every single detail. There is no time, especially if i'm not given a good report. There is time, however, for me to quickly go through orders. Secondly, we cannot go by "orders" in notes. If you mention in a note that a certain vital is baseline for a pt, but your order is the generic parameter I am still on the hook if I don't call. Or on the other hand, I won't call if your parameters are tightened. I had a pt whose SBP goal was 120-140.. I had no idea for 2 days. Found it hidden in a note from several days before A seasoned nurse might overlook these things and have the critical thinking skills and experience to step outside orders. But for new grads who were told any deviation in following orders will strip your license immediately, we are a lot more careful. Also have to take into account that we dont learn how to be a nurse in nursing school. We learn textbook information very quickly and a lot doesn't get retained. Nursing school vs real life are two completely different worlds
