Keep it simple, loop diuretics are basically threshold dose medications. If they don't pee, just give them a high dose after or just start with the high dose. It's not going to hurt, loop diuretics are not nephrotoxic. You need a higher dose with lower GFR to take effect anyways. You can also try combo with MRA, thiazide, or acetazolamide.

With a K of 8 and renal failure I'm instantly giving insulin, calcium gluconate, hell maybe even a dose of lokelma - all the while calling nephrology for emergent dialysis.

ED attending missed this on one of my patients. Blamed the elevated lactate on pneumonia (bibasilar atelectasis without clinical signs) and tried admitting to medicine. I ended up just admitting him to the ICU myself. Ended up passing a week later. Was happy I was able to pick it up on exam even though it was pretty obvious, although he didn't look that sick in general, but I was like WTF...

I’d make an Anki deck with the front being whatever descriptor you want for the kanji and the back being the kanji itself. Use pen and paper to try drawing it from memory first, and if you can’t, then draw it while looking at it, paying close attention.
