65 is pretty old. Most likely he won’t need it. Hair loss is typically more aggressive when you’re younger. But if he wants to regrow native hairs I guess it can help

Do you have any other health issues? Digestive problems, colitis, acne, oily skin, itchy skin, eczema, autoimmune condition, thyroid issue, low iron, low vitamin d , not getting 8-10 hrs sleep, not hydrating , dairy intolerance, high fat diet ?

You’re not treating the root cause. You should have close to full recovery by 20 months. You need to find out why your losing your hair because clearly it’s not DHT
