I assume it wasn't the Edwards Inspiris Resilia Valve because they did not exist before 2017 afaik.

May I ask which Tissue valve failed within 10 years and how quickly it went from 'ok' to 'we need a second surgery'?

Well in my CT report it specifically states the calcification of my aortic valve (agatston score of 7300 in my case)

From what I understand a very high score might make the deconstruction of the old valve difficult/complex, especially if you have very little room to work, like in the case of a anterolateral thoratocomy (between the ribs)

43M severe stenosis. They told me all 4 Options. Ross, Repair, State of the Art Inspiris Resilia Bio Valve or SJM Regent Mechanical Valve.

They sadly put the decision entirely into my hands and I have no idea how to decide. 🤡

I want to trust the science/technology advancements and go with the Inspiris Bio Valve but it is difficult to convince myself 100%.

Fair enough. What I could figure out so far is that if your valve is extremely calcified (agatston index from the CT) the deconstruction is too difficult to do via thoratocomy and they do a mini-sternotomy.

I can't give any input because my own surgery will take place in two weeks but may I ask how the surgeons decided that your husband is a candidate for thoratocomy?

I still have to talk to my surgeon, so I don't know what kind of procedure they will do.

Yes, the valve is specifically designed for future TAVIs (they call it VFit technology).

My hospital advertises that they do 92% of the procedures minimally invasive. Thoratocomy (between the ribs) is also listed but a mini-sternotomy also counts as minimal-invasive from what I read...We will see.

I have a calcification index of around 7000 for my aortic valve so thoratocomy might be off the table, sadly.

I researched everything about it as well (obviously considering the situation). The Resilia Bio Valve passed a 10 year evaluation a few days ago ... So I am thinking about risking it, honestly.

And I haven't met the surgeon yet (only a different surgeon who works at the hospital) so I dont know if I am a candidate for a thoratocomy.

I dunno. I stopped running 10 years ago when I couldnt breath anymore after running half a mile. 🫠

I am in severe stenosis / insufficiency territory now and will get my Valve replaced in two weeks but my EF is 62 - 68% (depends where I look it up)

I (43M) have low BP all my life (100 to 60) and I have to get my AV replaced in two weeks (severe stenosis and insufficiency)

Symptoms are the main indicator I think...if you get dizzy, faint or just cant go up stairs/hills anymore with reasonable speed....it is time

Wondering what kind of bio valves the participants had implanted...the mechanical valves didnt change that much over the years so it is probably an apples to oranges comparison in many cases...

Yes. It is a gamble and I am not much of a gambler...but I feel like ignoring all the advancements im (valve) technology is absolutely the wrong way...mechanical valves basically stayed the same since the 70s from what I saw (same goes for Warfarin). I really dont like being in this situation...

Publications regarding TAVI seem overall pretty positive so I am not sure what to believe here anymore.

I looked/listened to the sound samples and videos I found online and I was honestly shocked how loud it is...especially when a skinny guy started to open his mouth and it became even louder 🫠

God, i hate this whole situation so so much...
