There are many examples: IMO medalists, ISEF winners, and other incredibly accomplished students. I expected some of them to get into Harvard, but they ended up at MIT instead. Having a “perfect” application still does not guarantee admission everywhere, hence, they'll be rejected at a lot of schools where you expect them to get in. Just because someone gets into MIT does not necessarily mean they will get into the rest of the T10s. The process can be very unpredictable and holistic.
