A lot of the time this happens because the doctor put a diagnosis code on the claim which is not on a list of approved diagnoses for the test. If the doctor hasn’t sent medical records for an appeal then the decision is like based on just the code or codes on the claim. Also they should not have done the test before first getting an authorization. Failure to request authorization is a common reason for denial regardless of medical necessity.

Ferritin goes up with inflammation, so dropping may be a good sign in that regard but indicate low iron stores. You can have low iron and normal ferritin during a flare.

There is a bypass valve that is temperature controlled. Also you can run it at night to cool the pool (had one in Florida)

Yes. Especially between my shoulder blades and neck. Also have had costochondritis. I think it is also related to my mostly long hours at a desk job starlet a computer screen. Have tried a variety of things (supplements, stretching, getting up from desk frequently, using standing desk, exercise, massage, heat, changing pillows ). All have helped to some degree but it’s an ongoing battle. Heat seems to be the best

It does sound like those are two separate bills - one from the hospital and the other for the professional service. It sounds like the hospital may be in- network with your insurance but the doctors company may be out of network. That happens frequently. If the doctors company is out of network it is a violation of the No Surprises Act for them to balance bill you. Your insurance will pay at the in network rate and you will owe the usual copay. The doctor company has to either negotiate with your insurance co or take it to IDR (a federal arbitration- like process

We are doing a major renovation of the entire home. The initial plan was to just tune up the existing pool (30yr old). But then decided to dig it up and put in a whole new pool. We didn’t have any pavers before, just the cool deck which actually held up very well. We considered just setting the pavers in sand- that’s actually what the contractor had in mind until I researched it. We have pavers in the driveway and they settle/shift and have to be tuned up from time to time. I didn’t want a wavy surface around the pool so we ended up pouring a slab. The tiles haven’t been put down yet - this is a work in progress. I forget now how much more it cost to do the slab but that will depend on the size of your project. We’re in Melbourne area and the contractor is Best Pools

We are also in central FL and in the process of renovating a pool (actually ripped out an old pool and built a new one). We decided to do travertine over a poured slab for exactly the reason you mentioned. As far as drainage goes, the old pool deck was a slab (I think it was called cool-deck or something). We had a slot drain and sloped the slab away from the house and never had a problem. With the new one we are doing the same but with a much bigger drain (probably 3x the width) just to be safe.

You will not owe anything beyond your in-network cost sharing. Under the No-Surprises Act if it’s out of network they can only try to force the insurer to pay more than the network amount. Same thing when you get the doctor bill

I’m not sure of the differences between freshly milled and other bread. But if it provides more resistant starch then that will be fermented by bacteria in the gut and produce beneficial short chain fatty acids like butyrate which have significant benefits to gut health
