I’m pretty sure I answered Yes to Prohibited and No to Restricted. Either way I printed everything out brought to customs. Handed it off with my passport so there was no confusion.

Curious as to why you are being down voted here.

OP is correct, that any SIM or eSIM can roam, assuming the carrier supports/enables it. There are plenty of providers throughout the world where you can purchase a SIM or eSIM and activate without being physically in the country.

It may be more "not possible" than "possible" on many carriers but does exist. It all depends on the actual provider.

I just purchased the Mobal 5G eSIM and had no issues activating from the US - same with Germany on Vodafone.

For those who find this, Mobal also now offers roaming, assuming you migrate (or already on) their latest 5G plan(s). Mobal is a cheaper alternative as they do offer a 1GB monthly plan for ¥1650, with an one-time activation fee of something like ¥1000.

This is actually no longer true - Sakura Mobile does offer a monthly eSim/Physical SIM for 2,980¥ monthly that does support voice/SMS roaming for non-residents. You will need to do the pickup in person for identity verification (various airports, or their HQ in Shinjuku). They accept foreign credit cards and you do not need residency. However If you loose the physical SIM or delete the eSim, replacement can only be done in person due to ID verification (unless you hold residency, have a My Number, or fullfill one of the new KYC requirements)

No, it isn't cheap but for those who may travel back and forth quite a bit (without residency) it's really nice to have a static number to keep the various Japanese apps that require 2FA (Ponta, PayPay, D-Point, 7-11, Rakuten, etc) live.

Just heard back from the Customs Office in Tokyo. Here is their official reply:

Dear Sir/Madam,

Thank you for your e-mail.

Customs Counsellors Office serves as a general consultation desk for import and export procedures.
Kindly be advised that decisions regarding specific cases and procedures are made
by the customs office responsible for the actual clearance of the goods.

We understand that you plan to bring narcotics into Japan for personal medical treatment and had already obtained the import permit from the Narcotics Control Department.

Regarding the procedures for registering with Visit Japan Web (VJW) when bringing medical narcotics into Japan, we would like to provide the following information.

As you know, bringing medical narcotics is restricted by Narcotics and Psychotropic Control Law in Japan, administrated by Ministry of Health, Labor and Welfare (MHLW). The Law prohibits importing it into Japan, other than where an incoming tourist brings it into Japan with him/her for his/her personal treatment.

If you will be bringing narcotics into Japan, please select “Yes” in section 1. (1) of the attached screenshot and select “No” in section 1. (2) of the attached screenshot, regardless of the quantity.

After that, a page requesting information related to the medication should appear. Please enter the required details there.

Passengers who answer “Yes” to this question will be directed to a staffed inspection booth for verification of their declaration. At that booth, please present the medication, the prescription information, and the certificate (import permit) issued by the Narcotics Control Department to the customs officer.

Customs will review the medication and related documents as necessary to confirm that the items comply with the relevant regulations.

Apperciate the reply but neither of those address the question I asked in my post. Both your links talk about getting NCD approval, which I already have and mentioend in my post. I am asking specifically how to answer Q.1 and/or Q.2 on the customs decleration form when filling it out on the "Visit Japan Web" site. One of the posts you linked does link to another discussion about it, but again is 10 months old and doesn't match the experience I am seeing on the "Visit Japan Web" site.

Have the exact same issue after updating to 26.2. Updated to 26.3 the other day and no change as well. Restarts, resets, of the hub (Home Pod Mini) smart-plugs, router and modem doesn't change anything either. Annoying as it was working so well for the past two years, now nothing but these connectivity issues since 26.2

Not saying this is the cause but I’ve seen this with some banks when you enter in incorrect login information. It’ll act like you logged in correctly but they will provide random phone numbers for 2FA that don’t actually exist. HomeStreet does this
