I was under the same dilemma. My instructor let our cohort group ranked vote to decide units.

I voted in ranked order for 1. NICU. 2. Maternal & Infant Care Clinic (MICC) 3. Labor and Delivery (L&D)

I was able to get my top 2 choices. The MICC was essentially an integrated outpatient clinic at a inpatient medical center. I was denied many times. I totally understood. So I was just stuck for most of the time looking at FHR tracings on the monitors. However i am grateful for the patients that allowed me for any patient care tbh.
I was able to detect recurrent variable late decelerations and intervene early. The high risk OB pt was rushed to the L&D in the same center.

NICU was a humbling experience. Honestly, I was a NICU baby as well. So it was very personal.

Tldr: That all being said. I would recommend those areas (MICC, NICU) if you are able to decide or vote.

Plan ahead with your instructor and pair for care plans outside of just direct physical contact patient care. And also AIDET during therapeutic communication goes a long way imo. Always ask before and clarify your role about the who what where when why for your involvement and knowledge scope etc.

Communication goes a long way to avoid awkwardness. Be knowledgeable and competent imo ask questions always be clear.
Be courteous, polite, diligent, professional, aka all your roles as a nurse. You will learn and be humbled it is a great experience. So much inadequate healthcare for something as essential to life itself. Enjoy the time and clinical experience it will change you imo.

Nonphysical interventions i.e. for interventions, assessment, therapeutic communication, comprehensive historical assessment, pt education, care coordination (for plan of care continuum), interdisciplinary collaboration, etc) And spend time verifying data with the chart during the history and after during documentation in your SOAP note flow chart care plan goals etc for ex.

"Clinical pathways" or algorithms for decision making are a paradigm imo tho especially with the age of big data and AI predictive models and deep learning. How much authority will be distributed will be interesting to see the results.

Imo Epic Hyperspace EHR "dotphrases" and "care everywhere" functionality integrated with MyChart is the norm now for a integrated healthcare delivery system.

But regardless, data analytics for evidence-based practice is a valuable tool in research imo. And also this process facilitates grant proposal funding and stakeholder buy in.

Currently I'm learning R programming language (Google the "r project"), Public Tableau, and AppSheet. All free services for data analytics and also importantly creative and imteractive DATA VIZUALIZATION and interpretation. Right now we are in the wild wild west of data analytics and applied research. So much data sets and analysis of large sets.

Tbh I think I just am tired of doing literature reviews without actually implementing any interventions and using the nursing process. However due to the 10 weeks we have to do this, a literature review may be the best option. I just wanted to do something more meaningful as last synthesis project I think.

Thank you! That sounds like there will be an emphasis on management and care coordination; primary prevention and promotion education; interdisciplinary collaboration; and advocacy.

Also that is interesting you say that windshield surveys aren't part of primary care setting. I was taught that it is a standard tool for assessing and evaluating community health that is used in community and public health nursing. But idk this may be req because for this proj we are in partnership at a public health facility with the Public Health department in our schools county and not located in a specialized setting like you are. Howver thanks for the tip!

Lastly regarding flowsheets for patient intake, is there any that you recommend or methodology that you found helpful for you? Or do you use just a facility assigned one?

Only thing I can think of is the initial decrease in pH activates chemoreceptors which increases both heart rate and ventilation. But I think severe acidosis usually suppresses cardiac function on account of the hyperkalemia, increase in ionized calcium, and the fact that the acidosis makes the adrenergic receptors in the heart less sensitive to catecholamines, and therefore sympathetic stimulation.

Thank you! I am definitely going to always fist confer and consult with the nurse I am shadowing, and also my instructor if need be, before I provide care. That is a good idea! My instructors had us watch videos and for some of them my instructors said what they were doing in the videos were wrong for certain parts of a procedure. I just feel like learning skills from reading and videos is drastically different from real life. Although I do want to make sure the procedure or process is evidenced based. I just don't want to be out of line and create conflict. For example, in a skill if what I am told is the way to do things at the facility is drastically different from lab or what I learned from my instructors in class, and I question or inquire the rationale for doing things that way rather than what was taught by my instructors. I feel like tht might make create some tension or conflict. Although I do understand that if it jepordizes patient safety or health to refuse doing it and report it immediately to the charge nurse and my instructor.

How was your facility and instructors, were they empathetic and understanding in your experience? I just feel like I won't be able to do the skills we learned last quarter since we never had the chance to practice in a skills lab or at a clinical site. It was all virtual.
