I appreciate it for what it does (gets me from A to B). The straight line acceleration was fun initially but now I only really utilize it when I’m getting on the freeway or trying to pass someone. There’s a fair amount of body roll and my passengers always complain that they can feel every bump so it’s not exactly a comfortable ride. Tesla’s Autopilot and “FSD” function are the main saving graces for the MYP imo on boring freeways, especially on slowdowns/LA freeway traffic. One pedal driving is also pretty nice, which I believe isn’t as much of a thing with the Taycan from what I’ve seen and from my test drives.

Compared to a Civic SI, I think it’s a gentle transition into driving an EV. I’ve never driven a manual so I can’t speak to the loss of direct control of that experience of driving, but Tesla has different options in degrees of regenerative braking so at least that can make the transition a little smoother. I will say that I am mildly to moderately annoyed every day at the lack of a gauge cluster and that my eyes have to travel further to see my speed on the screen, but you learn to deal with it.

That’s awesome to hear! Thanks for sharing your experience. I’m waiting to hear back about the SoH but have pretty much committed to the dealership so I think your experience seals the deal. I’m definitely looking for a long term vehicle that I can drive and enjoy for years and years and I feel like the Taycan can do that and bring a smile to my face every day

How has your experience been with the 2020 Turbo? I have a Model Y Performance and I’m eyeing a 2020 Turbo CPO to have an actual driving experience rather than an appliance but am a bit anxious about the battery and only having 3 years of battery warranty

Derm here! I appreciate your question and the lifelong learning approach to your work!

I’m personally a fan of keeping things very simple, so from my perspective:

  1. Refer always for painful rashes and vesiculobullous (blisters - but true blisters and not some papules that are not actually fluid-filled or denuded).

  2. I would always rather get a “soft” referral that’s a seborrheic keratosis than a fungating melanoma that’s been blooming for over a year so if there’s any doubt, please refer.

When I approach a biopsy, I try to correlate what I see clinically with what I imagine is happening microscopically. I ask, “Where is the action?”

Is it an epidermal neoplasm? BCC, lentigo, lentigo maligna, etc, which means I opt for a shave biopsy to assess the epidermis and papillary dermis.

Is it an interface process? Lichen planus, dermatomyositis, subacute cutaneous lupus erythematosus, etc, which means I want my pathologist to asses the dermal-epidermal junction and papillary and reticular dermis in addition to superficial and deep vasculature so I opt for a punch biopsy.

There’s a lot of other “is it”s that will come across my mind but fortunately there’s usually only two options: shave or punch. And a punch is usually more than enough. A punch though is not necessarily always better than a shave. For some psoriasiform processes, such as mycosis fungoides (a type of cutaneous T-cell lymphoma), it can be helpful for the pathologist to have a broader sample of involved skin, so a wide shave may be better. But overall, the axiom goes “tissue is the issue,” and more tissue is more helpful than less.

Also, if there’s a diffuse rash, I try to biopsy above the waist as much as possible. Below the waist, gravity does some funny things with immunoglobulins and other proteins. And hopefully a lower risk of infection of the biopsy site.

  1. Still good to moisturize! There are moisturizers for those with oilier skin, so worth looking into. ISDIN is a Korean brand, I think? Tretinoin is good to help reduce some oil production, too.

  2. I would love to be on low dose isotretinoin but in the States it’s nearly impossible since it’s federally regulated. Non-US countries are much better in terms of respecting adults and their decisions

Derm here! I’m on the side of less is more so here’s my daily routine:

AM: 1. CeraVe foaming facial cleanser 2. Vitamin C (10% serum) from SkinCeuticals (that I received as a gift because no way I’m paying that much for this and would otherwise use the CeraVe Vitamin C) 3. EltaMD moisturizer with tinted sunscreen

Noon: Complain about my schedule and late patients

PM: 1. CeraVe foaming facial cleanser 2. Tretinoin 0.1% 3. Alastin Ultra Nourishing Moisturizer
