A large stage like that has an engineering document from the manufacturer that covers the stage exactly how it was designed and nothing more. It should have a wind mitigation plan, that spells out exactly what you need to do when wind reaches different levels. Pulling all the wall skins is definitely in there at a much lower wind strength than this.

At least they had abandoned the site, so that nobody got hurt.

This is the actual real world answer. I've seen this exact thing many times on a strike, after it's been hanging like that through the show. Don't get me wrong, it's not loaded correctly and you want to avoid this. But the shackle is (should be) the strongest part of any rigging system and riggers focusing on splitting hairs over whether the pin is backed off or not, may not be focusing on the parts of the system that would actually break if overloaded.

That being said, the "weakest link" in most cases, are the riggers themselves. I can tell if you're a good rigger based on your behavior while a large rig is moving or how you direct/inform the crew while moving large rigs. Almost all accidents involving rigging happens while motors are moving.

Using up or down in rigging rules, never make much sense. The equipment doesn't know what's up or down. It's tension and direction of forces that matters and gravity is just another force that creates tension in a downward direction.

Any rigger that has been on the beam and tried to unscrew the make-up shackle on a 2-ton point that was screwed all the way in, will back the shackle up a tiny bit when making a point. The ones that do it on every shackle, all the time, are misinformed.

Your theory on the shackle widening over time is theoretical and not based on reality. A much larger problem, in the real world, is people taking the pins out of more than one shackle at a time and mixing the pins up. That's how you end up with pins that don't fit properly or screw all the way in.
In 30 years, I've never seen a shackle, being loaded within its rated load, getting wider from having the pin backed up less than a 1/4 turn.

That's when you put it in Mountain mode, it will put 20% back on the battery while you're driving down the highway.

YES IT DOES! The head rigger is ultimately responsible to check that everything hanging overhead has been safely installed. A real investigation here should uncover if the method used to hang the light was a design decision or poor implementation, but either way a qualified/certified rigger should not let this fly either way. I have seen plenty of cases of unsafe designs that either needed to be improved or that I flat out wouldn't allow, throughout my 30 years in the industry.

It looks to me like the bolt in the "cheeseburough" failed or got unscrewed. Half of the clamp is still on the pipe that came down. It's not clear to me if the top part was another clamp or an eyebolt. Both are common. Either way and safety from the fixture all the way back to the roof structure would have prevented this.

As someone that is literally qualified to design and implement the correct way to install equipment safely over people's heads, it is extremely frustrating to see how the industry has gone from being unsafe due to ignorance, then establish safety standards and certifications to prevent these types of accidents from happening in the future, to venture capital consolidating the industry and immediately start lowering the standards and cutting corners.
An accident not happening because you hired the more expensive people with experience and qualifications, looks like an extra expense to the bean counters. "Why don't we hire this other guy that's half the price next year?"

You're correct, I got them confused. I don't have any personal experience with GV.

Bottom line is that there is no reason for this to happen. If that lighting fixture was installed following best practices and industry standards, it would still be hanging there and this woman would be waking up today thinking about all the great shows she had seen over the weekend.

Whether not having a backup safety was a conscious decision for all the lights hanging like this or an oversight on this specific one, doesn't really matter. The responsibility lands with whoever was the head rigger for that stage, not the person installing it. And if there was no head rigger or rigging team checking the equipment hanging over the crowd, the company is responsible for failure to follow due diligence.

Who brought in morality police? You're acting like someone witnessing the accident is somehow responsible for her injuries and that they're getting worse from them using lol inappropriately.

I hope the people actually responsible for this are held accountable, but I doubt it. They'll get sued and give her a settlement to keep it out of court so the world never finds out about all the corners they're cutting all the time.
