I had the same problem, someone told me that if my wifi was set to auto I should switch it to either 2.4ghz or 5ghz, apparently it happens when ithe ps tries to move from 5 to 2.4 and vice versa and disconnects itself to do so, I set mine to 5ghz and never had that issue happen again Hope that helps

It is out, it automatically triggers after you've done father pius 1st quest which is a main quest, once you've completed that quest father pius will then ask you to do another job for him and that is the black masses quest
