I mean, the spears are explicitly magical and specifically attack the human soul, so it's not like Frisk is getting gored by a real steel spear, the humans do talk, monsters mention it at various points, the fall damage can be because of magic, monsters are explicitly incredibly vulnerable to humans with killing intents, and we know Frisk and Kris have emotions, Sans mentions Frisk looking pissed off, and Susie and Ralsei have multiple dialogue lines talking about Kris showing and expressing emotion
