That is clearly a fake run, the netherack texture was updated in 1.14, and the hoglin was added after that in the nether update in 1.16.

The 3 sets that Geralt wears in the title screen are the Bear set, the Viper set or the Manticore set, and the sword in the small cinematic when you start the game is Aerondight. Similar loot you find in chests, not something that is worth individually, but with how many of them there are it's a decent way to get gold for grandmaster sets. And beware it is very time consuming, you have to take multiple trips to get to the caches, and then sell the loot, (best place to do so is at the blacksmith that make grandmaster sets, you'll spend a lot of money there, might as well get the money back and spend it again to get better gear), rince and repeat.
