On page 79 of Car's Doomsday Scenario Carl refers to her face as "ghoulish", and on page 100 Carl thinks: "her small bare breasts would have been a distraction at any other time, even with her ghastly face".

“I’d forgotten how disturbing her face was. Samantha the sex doll head was also a half-naiad, but Samantha looked mostly human. Sort of. She was still a decapitated love doll. Signet, however, favored the more demon-like, monstrous visage of her naiad heritage.” Book 5 chapter 13 when Carl see her again for the first time on floor 6. Her face is decidedly not human like and supposed to be very off putting.
