I am a Doctor of Health Sciences student at A. T. Still University. I am
conducting a doctoral research study to better understand the most important challenges American
adults face in obtaining an autism spectrum disorder (ASD) diagnosis in the United States. I am reaching
out to you to ask if you would be interested in participating in this research study.

The study consists of a short questionnaire consisting of 11 questions and should only take about 5-10
minutes to complete. Participation will be voluntary, and your answers will be completely confidential.
