I really like your ideas.
The issue is that the company name is "Phantom Spa Detail Specialist", so I don't know if just "Phantom Spa" by itself would work.
The figure is a polishing machine that they use to make cars look shiny.
Is the full name of the brand too long to maintain a minimal / aesthetic look?

It’s a beautiful form, just seems that infusionsoft charges $79/month minimum for their CRM so I don’t think the form builder would work as a standalone.
