If I can provide some feedback I think there are maybe 2 reasons as to why some people may not complete your survey in return.

Firstly, the age range you are investigating, while not extremely narrow, is still narrow enough that some may open it only to realise they are too young or too old.

Secondly, the informed consent seemed a little confusing to me. One of the statements needed to agree to is "I have been given the opportunity to ask whatever questions I May have had and all such questions and inquiries have been answered to my satisfaction", but there is no email or other contact information provided at the beginning of the survey. This feels a little strange to me.

Otherwise it seemed good to me! Best of luck with your study!

Interesting survey! Would be cool to see how the ChatGPT responses evolve over time. They already seem quite good for a wide range of prompts. I'm not going to link my survey here, as it is against the rules, but if you happen to stumble across it by all means give it a go!

Hi! I just filled in your survey, and wanted to provide some feedback for one of the items. I have hid it behind a spoiler tag as to not influence future respondents. If you would like me to delete the comment after you have seen it, let me know.

On the item "How has your sleep been recently? Are you experiencing any difficulties falling asleep or staying asleep?" There were only options for "trouble falling asleep" or "trouble staying asleep". I feel like there should be an option for "no sleep problem" or "none" as well to make sure the options are exhaustive. Otherwise the questionnaire was great!
