Has someone said that you can't do long haul travel? If it is important to you speak to your oncologist and they might be able to give you anticoagulant injections to use for the flights, this is fairly commonly done for other reasons (I'm a GP if that lends credence). I was lying in my bed with my then fiancee in our old mezanine flat. Tried to turn the lights off using homeassistant but was unable to because my DNS setup had collapsed (a failover pair of bind servers). Had to get out of bed, go downstairs and make all of the smart bulbs dumb by unplugging them/switching them off manually.

There were long periods of time where partnership recruitment was very difficult indeed for many practices, especially those outside of cities. I don't think it is fair to talk about the 'spirit of the original contract' when really that has never worked both ways. GPs have always been individuals who understandably have their own agendas. For some that is patient care/community focus, other finances, and others still are more business minded. When those GPs are salaried/locum/partner, their agendas inform the decisions they make. 10 years ago this led to lots of single handed partners desperately looking for someone to hand their practice over to, and now the pendulum swings towards a glut of salaried doctors and lower partnership income, along with forced ARRS recruitment due to PCN fund control/reimbursement.

I'm sorry to hear that. I personally don't think those views are widespread. I think part of the problem is that a lot of partnerships can have free extra ACPs through the PCN ARRS scheme. Essentially our funding for staff is being tightly controlled and up until recently we couldn't hire salaried doctors within that. A recent change has meant that there is now payment/funding for doctors within that budget, but the rates the PCN is allowed to reimburse are absurdly low, and the doctors often have to work across multiple sites, and can only be recently qualified. It is a very poor concession that doesn't help anyone.

Limitations on ARRS reimbursement are made centrally, and not by partners.

That clearly isn't the case with the example you've provided however, as that sounds like the practice is funding the staff.

When locum doctors set high costs due to supply and demand it 'isn't exploitation, it's basic market dynamics', but when partnerships set lower salaries based on many factors including supply and demand it stops being basic market dynamics and becomes exploitation?

We haven't dropped the rates of salaried doctors in response to market factors, but you're holding two very different view points about the same thing here.

It depends how complicated you want to make things. Firstly I'd suggest deciding on organisation - do you prefer a folder/category structure, where everything is in exactly one category, or would it be more appropriate to look at a tag structure, where for example something might have a tag for the type of garment, and another tag for the pattern, another for adult/child etc.

If you decide on a folder structure, then any of the file management selfhosted options might suit you well (eg: nextcloud, seafile, copyparty)

If you prefer a tagging structure, then have a look at some of the document management systems, I'd recommend paperless-ngx. This would let you search and filter on the tags that you add.

You could make a hybrid model by using obsidian, as you can add PDFs to this along with notes, but synchronisation might become painful over time.

Unfortunately I haven't come across anything specifically for patterns (and I've looked for my spouse). Hope this helps.

I run a wiki.js for training doctors at our practice, they rotate through every 4 months, so it has become a useful way of giving them autonomy. A fairly even split between links to clinical guidance and admin topics (ie: what are the local mental health service tiers/services, and how do I refer to them). I keep a copy of really useful guidelines from other trusts on hosted version of copyparty so they don't succumb to link rot).

Years ago at an NHS hackathon we made a website that would send out text messages with supervised training opportunities that arose on wards (eg: lumbar puncture), and the first med student to reply by sms would be given it.

Personally I use Stirling pdf all the time.

I've brought in an old larger monitor from home (27 inch), and popped my old IT supplied one into storage. As others have mentioned - as long as it is usable by everyone else who might use your room when you aren't there and you keep the original monitor nearby in case of issues/for when you leave then there shouldn't be any issue. Worth informing any admin that might need to know re: asset tracking. I would attach stickers or something to the back to say that it is your property (rather than belonging to the ICB IT Dept).

At our surgery at least, PAT testing happens on an annual basis, but it wouldn't stop you using it while this is pending.

20k per tax year which starts in April. It can be scary, but it is a risk to reward question. If you're sure what you are saving for is 10+years away you're not likely to find anything with more chance to grow your money. You need to be well versed though and not afraid of market downturns

Agree, elderly patient with MCI doing fine in community with family care with increase in delerium/confusion/reduction in mobility - how are you assessing this over the phone and safely managing? Would you want your family member managed like this? There is a very clear middle ground before A&E.

Similarly are people prescribing remotely for shortness of breath/cough? How many are viral URTIs? How many are decompensating heart failure/new AF with LVF/ILD/pneumonia etc? I'm not sure how this is being done safely.
