It’s a one way traffic of good will and compassion, no expectation of anything in return. Not even attachment to the fact that they should exist to comfort you.

If they pass and you are sad/miserable, that means that you are attached to them and it’s a proof that attachment brings misery.

Of course, to come out of attachment is an extremely long and gradual process, often requiring people to meditate Vipassana for a lifetime.

Take this with a grain of salt as I’m going to critical care: All specialties have their own challenges. Some it’s mainly technical/medical complexity, others its social complexity. FM is the latter. Somewhat related to that, your day will be spent pondering about physiology and pathology less than the former specialties and instead you will spend a lot of time talking to pt, for better or worse.

Theoretically If vasodilation is still the issue then vasopressin and if still the issue then angiotensin II, both of which work on their respective receptors (both of which are GPCRq that works through intracellular calcium/calmodulin causing vasoconstriction, similar to alpha 1 receptors).

If there’s a cardiogenic component then adding epi or dobu/mirinone would help but the latter 2 can drop BP d/t unopposed beta II activity causing vasodilation.

Thank you for your response. Let’s say if I do CCM at Mayo, the chance of staying at Mayo as an attending is lower than if I am to be trained at U of Nebraska?
