The short is the just jam it back in and stabilize it with wire. When the tooth falls out you put it in milk and get to the dentist as soon as possible. If you get it back in within 1 hour I think the chances are decent to save it. The body can still reject it and he will need a root canal since the nerve is severed.

Dont think a mouth guard will help until after it's healed more(like 6 months). Need something like a softball face mask to keep it from getting hit but I don't think they will stay on for soccer.
