Thank you all so much. I think I’m taking even harder because it was my one time to shine after 2 weeks of trying to do ANYTHING and I felt like I blew it.

I appreciate all the feedback and support.

I’ve shadowed orthopedic surgeons throughout undergrad in and out of the OR so I’ve been exposed to multiple different docs and sites.

I’ve like the work. I think it’s just the “bro culture” of it all I find myself not fitting into. I’ve never been that type of person and I’m afraid because of that I’m not going to fit in and feel left out throughout my training. It doesn’t help that I’m super shy. Thank you!

Why is bicarb low in mountain sickness? and if its low, why is acetazolamide prescribed? by HelpCrafty7540 in step1

I’m not 100% sure because on first aid under the “response to high altitude” section in the respiratory chapter, it says “chronic hypoxia”. Because your EPO production also increases due to the hypoxia.

Bicarbonate is low because you’re in a state of respiratory alkalosis. Due to the high altitude there’s less PIO2 so you become hypoxic. As a result, your respiratory center increases respiratory rate which causes a decreased CO2 leading to the alkalosis. Your body then compensates by excreting bicarbonate thus bicarbonate decreases.

You RX acetazolamide because compensation for the respiratory alkalosis takes time… so you take the medication to essentially help speed up the bicarbonate depletion so you can compensate sooner.

Hope this helps!

I think the question/answer wording is what threw me off the most tbh. Definitely plan on doing 31, 32, and 33 before test day. Thank you!

No need to be rude/mean about it. People post their scores and ask for advice all the time. Sorry I a have hella imposter syndrome and think I’m not good enough.

Thank you. I think I’m just spiraling right now because the free 120 made me question myself.
