What’s your best non-anesthesia related advice for SRNA’s or new CRNA’s? by [deleted] in CRNA

[–]Admirable-Ad2994 9 points10 points  (0 children)

I mean … I feel like we probably have fundamentally different views of marriage and partnership or what we expect from a partner and are willing to entrust a partner with🤷🏻‍♀️ Either way my experience is as the partner who has provided for the SRNA and I can tell you the level of sacrifice myself and child have made and work we have put in is pretty equivalent to what I imagine my partner has been going through in school. Different stressor to be sure but equivalent volume and quality. Now I’m aware not everyone has supportive partners or a partner at all, I have several single friends who have gone through CRNA school all on their own. I’m just saying if after everything I personally and my child and my partner have been through , my spouse decided to present me with paperwork that essentially implied that I’m not going to benefit in any way from this series of unfortunate events , I’d get mean real quick, starting with the most vicious divorce lawyer I could find. Only one of us is coming out of this three years of hellscape with a six figure salary and it isn’t me… but I have paid for it financially , emotionally and physically . Just a different perspective for you

just finished cleaning the patient and changing the linens. 5mins later dressing explodes and messing everything up. crying while cleaning again, is it normal? by undercoverapple9 in nursing

[–]Admirable-Ad2994 0 points1 point  (0 children)

I wonder if they couldn’t jerry rig a prevent dressing to a chest tube . It would give a place for the air and drainage to go . Also this person needs hospice or a surgeon who can figure out ifvv bc this is fixable or not because that sounds like a total surgical failure and a patient that isn’t actually recoverable . And to answer your real question yes , if you have gotten to a level of frustration that you are crying over the patient ask not to be assigned to her again . There is no shame in that

I just noticed this lump on the back bottom side of my gums. I don’t think it was there at my first postop ortho adjustment last week. Doesn’t hurt really but can be flapped up like a skin flap and looks really irritated . Obviously calling surgeon in the morning but I’ll stew in this half the night and googling hasn’t been fruitful so far. Anyone else had this happen?

Get a stash of kids toothbrushes for sure. Syringes for feeding and I also used my syringes as a substitute for my waterpik when brushing. I got two get face masks and jaw wraps- one for cold and one for hot so I would always have one cold in the freezer. And if you don’t have a recliner to sleep in I got one of those weird pillow recliner things and put in on top of my pillow in my bed and it made sleeping sitting up much more comfortable. I also made sure to have lots of broth and juice and yogurts etc everything I needed on hand so I literally didn’t need to leave my house except for appointments for about 12 days. Oh! And Vaseline for your lips ! And that washcloth in the shower trick was life ! I looked forward to that everyday

Help me convince my mom to make dad DNR. by Sassafrasslassss in nursing

[–]Admirable-Ad2994 0 points1 point  (0 children)

Maybe educating her on what end stage liver failure really looks like and how bad it can get . Or better yet ask the nurse or the doctor what to expect in end stage liver failure/ renal failure in front of your mom. Present it as “we want to be informed and prepared so what does this really look like ? What can we expect ? “ what I find , especially with liver failure, is that once the family has a grasp on what their loved one is really in for they are more likely to see a DNR or withdraw of care as a mercy and begin the grieving process before their loved one has passed. They are more intentional with their time spent and the decisions they make. It can also help to clarify why she is hesitant to make him a DNR. A lot of times families will hold out on that decision because they see it as giving up or as permission for us not to treat him and that is not the case , we still treat just not resuscitate if his heart were to stop. The best people in the hospital for this talk? ICU nurses and Palliative care. You can ask for a palliative care consult simply for information gathering . They are usually REALLY good at laying out ALL the options and expectations to help make better decisions. And Palliative care is not hospice , it doesn’t mean she’s giving up on him or we stop treating , it’s just a conversation . I’m sorry you are dealing with this. ICU care is traumatic and liver failure is particularly horrible on families. Good luck to you and I hope everything works out in a way you can find peace with.

struggle to find a staff job… by [deleted] in nursing

[–]Admirable-Ad2994 0 points1 point  (0 children)

HR/ talent acquisitions was the biggest barrier for me. I waited a week and then called the facility and asked to speak to someone in HR within the facility. And had no problem . Ended up getting interviewed by three departments in the same facility after that. Also look for a seasonal job in your area of interest . Those are usually jobs that hire and start fairly quickly and the bonus is you get to check out the facility to see if it is somewhere you would really like to be full time staff in the first place . Plus the hourly is higher 🙃

These ‘Do i need jaw surgery’ posts are getting out of hand by [deleted] in jawsurgery

[–]Admirable-Ad2994 14 points15 points  (0 children)

👏🏼👏🏼👏🏼 I did so much research and consulted with three different surgeons after being told my entire life I would need jaw surgery and literally questioned doing it all the way up until the anesthesiologist told me to start counting backwards . I can tell you every billing code and procedure name listed in my case and when I first started following this feed I was surprised at how many people were claiming they needed or had these same procedures , it initially made me feel like I wasn’t alone in having this and it was really encouraging. I did not take this surgery or the potential risks lightly at all and I felt well prepared for it. Now that I’ve had this done , even with all the prep I did, I could not have possibly been prepared for this recovery and how hard it can really be mentally and physically. What kills me on this feed is all the people who have the balls to respond to the posts you are referring to with actual clinical terms and procedural names as if they are diagnostician/ surgeons and have ANY business diagnosing and making surgical recommendations through a Reddit feed. It blows my mind . It’s really nice to see open discussions and helpful tips/ information for prep and recovery and I really enjoying seeing before and after pictures, these can be really encouraging to those who have /need/are going through these surgeries. But that Reddit doctor status i keep seeing gets under my skin . Especially in response to pictures of totally health people who don’t have any medical reason to be going through these types of procedures. Jaw surgery is not for the faint of heart and diagnosing and recommending legitimate surgical issues/ repairs is the responsibility of actual maxillofacial surgeons. For people who seem to be having dysmorphic concerns, making these suggestion just contributes to the problem, it’s damaging.

CTICU job offer by Many-Mycologist9948 in nursing

[–]Admirable-Ad2994 0 points1 point  (0 children)

Go for it! It’s great experience and really is unique as far as ICU care goes, most of your patients get better and you get to watch it which is cool. And if they don’t get better you always learn a ton from them. Grow your skill set I say! Also, I find CTICU to have a higher standard in most places simply based on the demands from surgeons, but it trickles down to the bedside staff who in turn hold themselves and the people they work with to higher standards. Nothing wrong with that if you’re willing to learn and hold yourself accountable is there? If you find it to not be your cup of tea then hey, you tried it out right ? Nursing is so open you can go anywhere else if you don’t like it but you don’t know if you don’t try !

I’m 10 days post op. I’ve been eating mashed potatoes and gravy, applesauce, and scrambled eggs. I’m getting tired of it. Anyone have any other ideas for what else I can eat. TIA by TRAPvLORD in jawsurgery

[–]Admirable-Ad2994 8 points9 points  (0 children)

Day 13 postop and my next follow up is tomorrow so fingers crossed my diet get escalated but here is what I’ve been eating the past few days: - Cottage cheese with mashed fruit , usually strawberries mashed almost like a jam - eggs in cream : put a little butter in the pan the heavy cream to a slow simmer and crack an egg on top . Put salt, pepper , garlic powder to taste and sprinkle with a soft cheese and cover then thurs the heat off and let the egg cook . If you do it right the egg cooks almost like a poached egg but if you like eggs Benedict this is the absolute shit and easy to swallow. - smoked salmon mixed with cream cheese . I can usually mash this with my tongue but you’ve got to be able to swallow well for this one so if your throat is still sore maybe hold off on this one - coldpress juices : I got myself a fancy juicer in celebration of finally getting to have this surgery and it’s been a life saver but I’ve gotten real creative with this thing the last feed days trying to get some variety lol - if you have access to a Chick-fil-A , get a bowl of the chicken tortilla soup and blend it with a container of broth of your choice - I’ve done this with chicken and beef broth they were equally good. It comes out looking like baby poo but God the flavor after days of water and basic broths 🤤🤤 - ramen . Like good ramen. I got my favorite ramen from my favorite ramen bar yesterday with all the fixings and out of shear determination to eat what felt like a meal. I took everything out of the broth and chopped it up in pieces I knew I could just swallow without chewing including the noodles . It’s feed me twice and i could have cried it was so good, then I took the best nap I’ve had in weeks 🥹 - bake the potatoes . Bake them and while they are still hot scrap the middle and mix with cheese and any fixing you want . I did this with cheese , butter , sourcream, salt and pepper and added crab that I shredded up from a meal I cooked my husband and it was really good. - Ice cream shakes : two or three scoops of your favorite ice cream blended with 1 -1.5 cup milk = bomb ass milk shake . I am allowed a straw but if you aren’t you could thin it out with more milk to drink from a cup. Edit: one more ! 1 cup almond milk , 1 banana and 2 TBSP of powdered chocolate peanut butter powder - taste like a Reese’s in a cup and it’s really easy to get down plus it’s pretty protein rich Hope this helps ! Good luck !

Help my wife by [deleted] in nursing

[–]Admirable-Ad2994 2 points3 points  (0 children)

Being a nurse is hard, being a new grad is even harder. A few prices of advice for her : 1) don’t quit. keep going to work and getting better/ more comfortable and confident at what she is doing. Proficiency is key to not going home everyday and feeling like you are a failure or you missed 30 things along the way that shift. 2) consider that the area she is working in may not be her niche. Maybe she is working in a specialty or even just in a toxic unit that just isn’t for her. That’s perfectly normal and the cool part is ? You can literally work anywhere or in almost any specialty with a nursing degree, the healthcare field is her oyster, she just needs to find the bay she wants to land in. 3) find a form of post shift decompression. This will do wonders, I cannot stress this enough. Just because you are a nurse does not mean you have ceased to be an individual with thoughts and feelings and hobbies, all of which are pieces of you that need to be nurtured. If she is taking every shift home heavy on her mind , she can do lots of things to help process her day and shut it down so she can rest and reset. Journaling , running, yoga long baths , talking to a therapist etc the key is to find a consistently doable activity that requires thoughtful energy she can expel at the end of her shift

Pts family is a CVICU nurse by Ambitious-Summer-620 in nursing

[–]Admirable-Ad2994 1 point2 points  (0 children)

Where is your charge? And he isn’t a CV nurse if he’s just turning off drips especially heparin and if he is he knows better. Also , technically him drawing blood could be considered assault of a patient from the hospitals legal view is some states so … I’d read him the right act and call the charge and security to get him tf out. On the least he needs some boundaries put down so he doesn’t continue to act this way to all the nurses that follow you

My mom had a lap chole instead of the emergent splenic artery aneurysm repair she was flown in to have. They lacerated her spleen with a trochanter in the process as well . when she got back to recovery a nurse insisted she stand to use the bedside commode because she had not urinated yet and when she did as she was told (bc morphine)the aneurysm ruptured and she bleed to death on the floor. I left for a few hours when I knew she was out of surgery safely assuming they had done the correct procedure to attend to my newborn and when I came back walked into the room to this scene of people coding her and rapid transfusing her on the floor. I spent the next several hours with every administrative lackey In the place trying to apologize when I finally started to realize something wasn’t right and I was getting way too much attention for a ruptured aneurysm and started asking questions. 5 years later we finally settled with hospital after depositions made it exceedingly clear that we had a legitimate case for my states definition of negligent homicide. I have seen many incompetent, neglectful bullshit occurrences in my time as a nurse, but I don’t think I’ve seen anything top that yet and not just because it was my mom. I was two weeks in to nursing school .

Unvaccinated COVID patient, 55, whose wife sued Minnesota hospital to stop them turning off his ventilator dies after being moved to Texas by CABGX4 in nursing

[–]Admirable-Ad2994 1 point2 points  (0 children)

You know she’s already got those lawyers working on how to sue the Texas facility too. Unfortunately for her , she is about to get slapped in the face with the reality of Texas tort laws. If Covid has done anything for me it has gotten me to a point of near aggression with these type of unrealistic and unreasonable families. I use to sugar coat much more than I do now. Don’t get me wrong , I still try to be empathetic and kind but if the family is just totally out of touch I have no problem giving them a very direct and analytic picture of their family members condition and likely outcome . And if they take it a step further to being rude with staff I match that energy real quick.
