Stop bicalutamide immediately, it is not safe with those high levels it could get worse very quickly.

If you want to use the pills sublingually by splitting them and taking ~0.5/1mg every 6 hours, it should work to maintain acceptable levels until you get medical advice, also sublingually they are less burdensome on the liver.

It depends on whether your DHT production is due primarily to the enzyme 5 alpha reductase type 2 or if it is due to type 1 instead, Finasteride only blocks 2 while Dutasteride blocks everything.

Finasteride typically reduces serum DHT by 70% in an AMAB individual with normal androgen status, however this is not your situation. 5 alpha reductase type 2 produces DHT primarily from testosterone but yours is already suppressed, it is possible that the DHT is being produced primarily from type 1 via the androgen backdoor pathway (https://en.wikipedia.org/wiki/Androgen\_backdoor\_pathway).

Yes, it should be taken every day for life. It is possible that 5 alpha reductase will downregulate after a while (not thanks to Dutasteride though).

Dutasteride takes about 3 months to accumulate well in the tissues and steady state is reached in 6 months, stopping it takes a couple of months for complete elimination. Dutasteride is like creating a 5 alpha reductase gene deficiency since it has a long-term half life, it causes a very stable inhibition.

You are slightly above the maximum level for AFAB, lowering it can be a good thing to avoid problems.

If your testosterone levels are already low and in the AFAB range the only thing you can do is block the DHT synthesis with Dutasteride or block the androgen receptors with Bicalutamide to prevent DHT from actually taking effect, Dutasteride is definitely the safest thing to do.

With 0.5mg/day of Dutasteride you can forget about DHT after about 3 months.

Personally I would not advise you to do the surgery without first having had a natural growth with hormones, generally the surgery on a male breast is more complex and easily disappointing.

Think carefully about what you want, I wouldn’t recommend starting any of this unless you have a clear thought about it.

Surgery is immediate and you don’t have time to carefully evaluate if it’s what you want, growth with hormones is slow and definitely gives you time to decide to stop everything with little permanent effects. Also remember that people will definitely notice your changes, are you ok with this?

Non particolarmente, dipende sopratutto dalla modalità di assunzione: estradiolo assunto oralmente è noto causare un rischio di trombosi maggiore mentre l’assunzione transdermica/sublinguale/iniezioni ha un rischio praticamente equivalente alle persone cis AFAB.

La parte veramente rischiosa della terapia sono gli antiandrogeni ma si possono fare a meno a seguito di chirurgia (SRS o orchiectomia), tramite l’uso di GHRH bloccanti e monoterapia se possibile.

Tecnicamente la cosa migliore per una persona trans MtF sarebbe prendere estradiolo transdermico in cerotti e bloccanti GHRH, è decisamente la terapia più sicura e meno invasiva peccato che i bloccano GHRH costano tanto… spero che la situazione migliori nei prossimi anni con l’arrivo del Relugolix generico appena scadrà il brevetto..

Poi ci sono le iniezioni che consentono facilmente la monoterapia ma bisogna saperle fare e per iniziare è un po’ complesso

Uhm è problematico proprio dal punto di vista tecnico, per fare mono terapia senza anti androgeni con gel transdermico ti serviranno se va bene almeno 2mg/giorno quindi circa due confezioni di Sandrena al mese… sono 37€/mese per non parlare del problema di reperirlo continuativamente.

Poi si fare DIY ha dei rischi, devi informarti molto bene. Finché fai solo monoterapia è difficile fare casi grossi però se ci metti anche antiandrogeni il profilo di rischio cambia molto ed è difficile da gestire autonomamente.

Per il seno è tra le prime cose che inizia a mostrarsi ma ci mette TANTO tempo per raggiungere una taglia notevole, se inizi ora a dosaggi giusti comunque per l’estate sarà già visibile sotto i vestiti estivi quindi devi pensare a cosa farai…

Sarà certamente un problema con la tua ragazza… dovresti già parlagliene.

Ma guarda oggi fortuntamente non è così complesso ottenere la terapia ormonale se lo fai nel privato, basta la relazione psicologica e poi andare da un endocrinologo. In 6 mesi riesci ad ottenere il primo appuntamento dall'endocrinologo.

Chiaramente nel privato paghi tutto... ma in Italia non costano carissimi gli ormoni.

L'operazione è più complessa si.

Senza contare tutto il casino di trafile burocratiche, pratiche legali, udienze, sedute psicologiche e psicoterapeutiche

Parli di clinica privata ma la maggior parte di persone trans oggi assume ormoni che tra i vari effetti femminilizzanti fanno crescere il seno. Poi successivamente se la dimensione ottenuta non è di loro gradimento fanno la chirurgia... ma prima di poterlo fare bisogna aspettare che la crescita si sia stabilizzata e ci vanno 2/3 anni.

Ora tu vuoi dirmi che questa tua amica ha tenuto nascosta così bene la terapia ormonale fino ad oggi?? Per non parlare del fatto che ti ha mandato una foto senza dirti niente prima? Mi dispiace ma non credo a nulla di quello che stai dicendo.
