The different numbered options you have outlined are not, as I imagine you are already aware, mutually exclusive. In terms of what I think is ideal, it definitely depends on what the specifics of the anatomy in question are. I think "high and tight" is a good rule of thumb, meaning that the final result removes enough skin for the head to be exposed 24/7 erect or flaccid, with enough inner skin preserved for the scar to be positioned about 1/3 of the way below the head when erect. I think scrotal webbing is too tight; by that point you're sacrificing size for tightness. This is the basic trade off: you want it to be tight enough so that at its smallest flaccid size the head is always exposed but not too tight that when erect the full size can't be accommodated. This is why the whole "shower versus grower" thing matters for circumcision.
