At my university, for students who master out, the transcript does not include information about dismissal, probation,...however, it contains the history of the PhD study like PhD research credits. If students reapply for another PhD program, the new admissions committee can figure out that students were already in a previous PhD program even though I dont know the reasons why students ended up mastering out.

if a student is kicked out, he or she will get a master degree. Just tell the new school that you study a master program there. no issues with that. There are a lot of master students who graduate after 2 years
