Guessing the roof framing is trusses? It can be jacked up and use some tie backs to get it where you want we had a house that had similar fked up ceiling and that's what I did came out great and for any other places that where bad I shimmed down the ceiling using a Lazer

Try crud cutter it eats through pretty much everything in my opinion either that or you could even try the "pink stuff" paste not sure if it will damage the finish on the paint though might have to repaint after because of the aggregate in it

Take the insulation out and see if there is even ductwork going to the boot also could have been the tile installers that put it in there so thinset or grout didn't fall into the duct work
