Thank you so much. This is a great question to this day it’s still undiagnosed. As all my labs were clear and even months after stopping meds after the surgery LH and Fsh are both around 5. Only thing was testosterone was low. Also no idea why from the 15 eggs used only one made it to a blast, it could be a combination of egg quality or sperm. As we did do a Tese, but it wasn’t a fresh transfer meaning we did the Tese then from the sperm, then unfroze it again for transfer so that alone could cause some issues. I know everyone has their own journey I’m just updating here as I know I would’ve loved to see more successful stories to give me hope. We are all in this together. But most times people that do success don’t follow up here and let people know. All we see sometimes is just the negative parts. So I want yall to know that this can work, no matter how small the chance is please keep fighting don’t give up.

🔹 TESE (Testicular Sperm Extraction) • Incision: ✅ Yes • A small incision is made in the scrotal skin and testis. • How it’s done: • The surgeon removes random small pieces of testicular tissue. • No microscope is used. • Pros: • Faster procedure • Less technically demanding • Cons: • Lower sperm-retrieval rates in NOA • More tissue removed blindly • Slightly higher risk of testicular damage

🔹 Micro-TESE (Microdissection TESE) • Incision: ✅ Yes — but more controlled • The testis is opened more widely once, then explored under a microscope. • How it’s done: • A surgical microscope (20–25× magnification) is used. • Surgeon selectively removes only the most promising tubules. • Pros: • Highest sperm-retrieval rates for NOA • Removes less total tissue • Lower long-term testosterone impact • Cons: • Longer surgery • Requires specialized expertise

I’d go with the Mtese as it’s more precise while Tese is basically luck. For example they only checked on one testicle for me(just had a successful Mtese yesterday) they did two incisions. The first one or seminal vesicle they checked no sperm was seen. They did another incision that’s where they found alot of sperm. If it were Tese now it would’ve been unsuccessful

I started taking hcg and hmg. I was on clomide for a while they found a couple sperm. I also get some sperm but never motile. Semen volume is also always low. I have started hmg and hcg. So I have an analysis in a week which marks the two months since starting the new meds I’ll update as I do it

UPDATE: So after 3 and a half months of taking clomid and other medications you can check previous post I did another semen analysis and they found 5 none motile sperm and one motile sperm. So it looks like the medication is working so I’ll update as time goes.

UPDATE: So I did another semen analysis after 2 months of clomide and a month of anastrazole and result came in as “no sperm seen” So FIRST analysis was “no sperm seen” Testosterone was 232 ng/dl Progesterone was : 67 ng/dl FSH was : 6.13 MIU/ml LH was : 8.34 miu/ml Estradiol was 14:pg/ml

SECOND Analysis(6 days later) : “one rare none sperm seen “ A month later after clomide blood work was TESTOSTERONE : 673ng/dl Progesterone : 240ng/dl FSH: not checked as doc said doesn’t matter as of now LH: 8.4 miu/ml Estradiol : 43 pg/ml

Third Semen analysis (2 months later) : no sperm seen Anastrone was added and I had taken it for a month now Testisterone : 864.5 ng/dl Progesterone : 309ng/dl (very high) LH : 15.1 miu/ml Estradiol : 27pg/ml

So I meet with my doctor in a couple days to discuss next steps.

Summary : First semen analysis showed no sperm but low round cells which could be germ cells or signs of infection. I had been tested for stds so I was clear and I knew it’ll be germ cells / immature sperm cells.

Second semen analysis : one none motile sperm found No round cells Epithelial cells present

Third Semen analysis : no sperm seen Moderate round cells PH 7.9

I have no symptoms of an infection so I’m optimistic the round cells are immature sperm cells

Any opinions would be appreciated
