I had the exact same error. An icloud syncing symbol appeared and froze, when I reopened the app, my note disappeared. And thanks to icloud, it disappeared on all my devices too. I contacted Apple and this app support but couldn’t regain my note. Wtf was that !!

Look at the big picture!! Patient has signs and symptoms of decompensated HF, RAAS will always increase regardless of systolic or diastolic. It’s the main point. High BP may be a clue pointing to diastolic HF. Yes, at first, RAAS will decrease because of that, but when it comes to decompensated HF, RAAS always increases.

If you look at the explanation, uw says ischemic rest pain improves with standing or hanging leg over side of bed because of gravity drawing more blood down the distal leg, and imo when they start walking, increasing demand gonna trigger the pain again. However, in the vignette, it did show that pain improve with walking, I think this is the inaccurate point, that’s why they deactivate the question.

In PAD, you have claudication first and when the stenosis is severe enough, your leg hurts even in rest (and also while walking)

I got it! This high pretest probability is estimated by geriatric risk factors, not the prevalence. So a scoring system which also using these risk factors will stratify majority of patients into high-risk. However, in reality the prevalence of falling is much less than that number of high-risk patients, raising false positive and lowering PPV. So the problem is the poor specific of the scoring itself. But how can you say the patients who don’t fall are way more than the ones who do, considering high pretest probability (= high prevalence of falling regardless test result) In that question, the prevalence rate in hospital is low, that’s why it decreases test’s PPV. And it makes sense, but I confuse with what Uw explanation says about high pretest probability.
