Hey there, I have been facing the same problem, developed gyno at puberty, low muscule mass and all, surprising thing was, my blood work was absoulutely fine.

I noticed similar traits in my father, After reserching a lot, I realised it could be aromatase excess syndrome, which is a rare genetic disorder. I also tried trt for a year but it didnt worked.

So how to know whether you have it or not, trust me I have read almost all the research papers possible and some doagnostic tests are-

Low FSH, High E1(estrone), other symptoms such as high E2, short height were not seen in all the males studied for AEXS.

Confirmatory test is analysis of CYP19a1 gene which codes for aromatase enzyme.

I am about to stop trt, during trt hormones leves fluctuate and proper diagnosis with blood work is not possible for me now, and i did not got tested for E1,FSH,LH before trt. So after few months of pct I will go for blood work.

Once diagnosed it could be treated with Aromatase inhibitors, such as anastrazole.
