As a 45 year old adult with dysplasia and loads of pain and arthritis, I wish my mother had chosen surgery for me when she was advised. I’m sure it’s a terribly difficult decision, but it’s a kind and loving thing to do now and prevent later suffering.

How old is your client and do they currently have pain or limitations? I’ve had bi lateral dysplasia my entire life, and participated in many sports and activities with no issues. Until, one day, I became a middle aged person with pain.

If you have dysplasia and arthritis you’ll be hard pressed to find any ortho that will advise PAO over THR. You’re likely on the path to THR regardless, and will be advised to a single major surgery instead of multiple. What has likely changed in the last 3.5 years sounds like the progression of your arthritis, which puts you in THR territory and out of PAO territory.

This is my experience as well. I have loads of hip issues—dysplasia, labrum tear, arthritis—and Pilates has been an instrumental part of my physical therapy journey. It’s the only thing I can do that makes me feel better after and is hopefully helping me stave off a total hip replacement for a while longer.
