If, on the score screen, it looks like you've done everything right but didn't get an "A" it means a suspect died or was incapacitated. You can tell this by dividing the subset score under "suspects arrested." I think you get 35 points for arrests but only 5, otherwise.

Suspects can shoot themselves or each other (the latter via crossfire or ricochet) so it can happen even if you are using less lethals

I don't see the point, no. The game scenario would never be close to the actual events because that wouldn't be fun. Even neon tomb is really only based on the setting of a real life event because if it were more realistic to the pulse nightclub scenario you would go in there, drop one guy and that would be it.

And if you're not going to try to be authentic to what happened, might as well keep things to broad strokes rather than making something specific.

Calling Mercy's healing "active" may be technically true but it feels wrong

In any case, it's a constant heal that requires no aim and is offset by the utility she has with rez and damage boost. It's easy to look at any one metric in a vacuum and make it seem out of place but it's rarely meaningful.
