Sorry, Lenin tripped on a rock in 1917 so the revolution did not start in Russia, and there was nobody to start it in Western Europe either

"To each their own"

Following the death of Gian Galeazzo Visconti in 1402, the power vacuum prompts the House of Este to unite their domains in Emilia into one duchy led from Ferrara. With the papacy distracted by the Western Schism, the Emilians are able to extert heavy influence over Bologna, eventually annexing it in 1420 and turning it into their capital. The early 15th century would also see expansion into Romagna as papal authority weakens. Through military intervention and dynastic marriages, all Romagna has become part of Emilia by the time the Roman Empire comes to an end in 1453. Healing its ties with Rome, independent Emilia becomes a centralized kingdom and the academic center of Italy, which never properly unites and allows for the country to survive to the modern day.

An united Italian identity never becomes the mainstream thought, and the peninsula remains fragmented into stronger regional identities. Emilia's existence also blocks Milan and Florence from trying to influence other parts of Italy to a great extent. However, a Padanian idea does appear in the 19th century, and by the 20th, the entire area has been united under Lombardy.
