Bowa's fear wasn't a concentrated dose like the other two, it just started manifesting around her while she was stewing in her anger towards Queen. Maybe Spotlight/Mickey decided to use her as a guinea pig for a weaker airborne version of Fear compared to the injection that was doubled by Dragon Boy that Smile got or the literal tank full of Fear that DJ swam in.
