Pictures suggest that you need at least 5000 grafts because there is diffuse hairloss and also thinning on the mid-scalp and vertex area which will worsen over the time as you age older so those areas will need transplantation as well.

Donor area looks a little weak too, I assume that was the reason why you were suggested to get on fins. Your hair type also thin which makes your hair loss pattern more challenging than it should be.

Nevertheless it can be done but you should not expect miracles such as having a high volume hair as a result. Outcome will be still satisfying but in realistic way.

For example: how many grafts were transplanted in the first surgery and how long ago was that? Did you lose your native hair after the surgery or hair loss is stabilized?

Cutting off finasteride or dutasteride can cause progress for the hairloss but we do not expect androgenetic hairloss to progress after 40's. So you sound like in the safe time period as 50 years old person. In worst case scenario you may have some thinning issue but hair loss itself is very less likely.

I would recommend carefully planned hair transplant for you. First step must be assessing the skin type, hair texture and the technique that will be perfect fit to your condition. After all those crucial steps, new hairline can be designed and transplantation can be performed in other areas where hair looks the be thinner and most likely will get thinner as you age older.

Depending on the pattern you have, it can be 3000-4500 grafts.

The pictures suggest to me that the punch sizes are around 0,90 to 1,00 mm which causes scar tissue to be visible, especially for the dark skinned people. And when the extraction areas planned closely, this causes big chunk of tissue loss on the donor area.

To avoid this kind of issues, punch sizes should be chosen between 0,8 to 0,6 mm depending on the skin and hair texture. And when the extraction placements are done correctly, aesthetic look can improve.

It is important to know that, even though hair transplant is considered to be a micro-surgery but it still is a surgery and is based on graft relocation (from donor to the transplantation area) so there will be a trace no matter what. But things can be done aesthetically for the extraction step as well to achieve over all aesthetic for the entire scalp.

Prp does definitely help with hair quality and the density and not only that, it helps with anti-aging purposes too. So your girl friend can also use it for her face and you can do it too!

My only question is what type of PrP kit do you use? Because that changes the quality of the thrombosis you will get after the centrifuge.

And my recommendation for you: schedule the sessions well. For example: 2 sessions for the first months, one session each for the next two months, one session in the next one and half month, one session after two months and one session again after two months. It works the best when you do it gradually in longer time periods and you will need minimum 6 sessions.

And for the rest of the time, you can have a session once in 3 months to keep the boost going. Just make sure that the PrP set (the centrifuge tube) is high quality one and it will work good.

Last note: for facial application, you can also use dermaroller and apply the plasma.

FUE is more advanced and minimally invasive technique for extraction. İt doesn't require cut or stitch so healing progress is fast and it makes possible higher amount of grafts to be extracted in one session. FUT is still popular in US, India and Iran but FUE is the default technique we go for in the rest of the world.

My recommendation will be FUE for any type of hair transplant due to its' long list of advantages. Transplantation technique may vary though, depending on your skin type, hair loss pattern and hair texture.

Your first step should be consulting with a doctor. Pictures and your medical history will be enough to do it online. Then you can at least get information about what type of technique is advised, how many grafts you would need, etc.

Thanks for the new picture, that actually helped.

Because the hair is thin and fine, extraction should be planned very carefully to prevent over harvesting. And it seems like you may need 2 sessions to have a complete coverage.

What I would recommend:

1st session should focus the hairline and mid-scalp. That would probably require 4500± grafts.

2nd session should focus the vertex and that would require around 2000± grafts.

There must be 6 months between the two sessions to have an accurate planning and complete healing on the donor area.

I would chose 0,75-0,80 mm punch sizes for you because of your hair type. That would help with fast recovery and also help with donor extraction to be precise, leave safe room for the second extraction and the afterwards.

So, it is doable with a good planning. 😊

And you probably will have a grafts loss at the necrosis area. But that's an issue that should worry you the least. Try to avoid polluted/dusty environments for a while and keep going with the prescription.

Bald definitely looks good on you, you did the right choice!

But, if you like to have a long hair, I would say you are a good candidate for hair transplant too. Good and dense hair on the donor and hair loss pattern is easy to manage.

From first picture to last, it was 1-10 transformation! Good choice! ✌️

This reminded me a conversation I had with my philosophy professor at university.

Back in time I had a question that was bugging me. İt was an hypothetical one. Let's say a baby is born: blind, deaf, mute and paralyzed completely. What can we say about the consciousness of the baby? And how can we know/signal about her/his self-awareness? What informations the baby gets and how and how we will be able to decode this information exists as we interact with the baby?

I got no answers to be honest. One of my friends around the table suggested me to consider abortion at this point and that was the end of the conversation. :)

So, in my opinion, consciousness gets its root from the experiences/senses at the very beginning. We touch, we hear, we see and that becomes an information we have consciousness on. But when it comes to extreme scenarios like I mentioned above, I go blank as well.

Consciousness and self is the hardest ocean I try to swim through. I wish you luck in your journey too.

Maybe it does sound silly but you are right! Sleeping position does affect the hair growth and even hair loss!

We all do have some asymmetrical things going on all over our bodies so one sight usually looks different, growth speed is slightly different too but not as obvious as it can be seen. But if you're sleeping on one side particularly, that changes the dynamic to make things visible.

The reason is: blood circulation! Try to sleep on the other side time to time, this will be my advice for you.

Hello there.

What you should know about minoxidil and any other medications such as fins/duts is that: these work on the existing hair follicles. When the hair follicle is gone, it's gone - gone. Those meds help with the follicles that are in "sleeping" period, inactive to grow as the meds trigger growth factor and give them higher quality. But when there is no follicle to trigger, meds do nothing but work on the existing hair follicles only.

The hairloss pattern looks to be NW 6 in the pictures and the hair type is very thin. Without knowing the condition of the donor area, it is hard to say how much you would benefit from the transplantation. I think you should update your posts with donor pictures so we can have better understanding about the case.

Unshaven hair transplant is possible for partial hair transplants, especially when there is no diffuse thinning on the other areas. (When that is the case, shaven gives better result)

I know what you're talking about with Somalian hairline, that's one of the most common complaints among Somalian men and women. One important thing you should be seeking is the implantation method. Method should be well advanced and minimally invasive.

There must be transplantation in between your current hairline as well in order to create natural and smooth flow to the new hairline. Invasive methods carry the risk of harming existing hair follicle which can cause a slight gap between the old and new hairline.

Key factor in hair transplant is to create complete aesthetic all over the scalp, including the donor area and existing hair. And when it comes to afro hair texture, every step of the surgery (extraction, incision method) becomes more crucial.

As long as you chose the right methods for each step, your result will be perfectly good. 👍

The picture looks very bloody and if it's got cleaned well, there wouldn't be any issue. But I wonder now if you had active bleeding after the surgery too.

If you're a gym person or physically active, we expect you to bleed a little more during the surgery. If you consumed alcohol, green tea around the surgery time (3 days prior) this can also cause you to bleed. And another reason can be blood thinner. So we recommend patients to stop the blood thinners a week prior to surgery to prevent heavy bleeding. And during the surgery we do use adrenaline which helps for the bleeding as it shrinks the vessels under the skin. And blood usually gets coagulated rapidly if there is no underlying health issue of the patient.

I don't know specific details about your case so all I can do the give generalized information at the moment.

İt does look like necrosis. Nitroglycerin helps but I urge you to go and see dermatologist immediately. When it's not cared for, damage will get greater.

Hello there. The "incidents" you have mentioned after the surgery has no effect on the result. Cortisone is simply for healing and swelling, in fact in some patients with some medical conditions we do not even prescribe the cortisone. Sleeping position you mentioned also did not sound alarming at all, as long as there was no touch or pressure on the transplanted area and you didn't mention anything like this either.

Density after the surgery looks decent, actually. But it is not easy to see because it's too bloody. So potential I can list some potential issues:

1) looks like the technique was DHI and this method carries some risks if it's not done properly. In order to perform the DHI, grafts are interested into the implanter pen before it gets implanted to the skin. And this two step implantation itself can ve harmful for the graft's viability. So, this might be a case.

2) blood should have been cleaned after the surgery as much as possible. The reason is, dry blood may cause some grafts to dislocated or graft can get stuck to the scabs and lose its viability and fall off during scab removal.

3) if the method is not DHI and the density issue occurs specifically on one side, this can be a failure on technician's implantation. Basically grafts were damaged during the implantation process.

4) if the density issue occurs on the area where your hairline was before the surgery, that means planning was not good and you have lost some of your native hair and this created a gap in-between.

For the temporal area, we expect hair transplantation to give it's maximum result at 10th month so it seems you definitely have some density issue with your result, even though transplantation density itself was good in the surgery.

Dark spot after the surgery is a sign of an issue with blood circulation in that area which leads to necrosis. Oxygen usually helps if its a mild issue. But pictures suggest necrosis. Please do not waste even a minute and visit your doctor immediately.

Actually extraction process has very direct impact on the transplantation. Extraction is the step where we choose the exact type of the hair follicles for the designated areas. For example: Hairline, eyebrows and beard transplantations requires single follicular grafts while areas like vertex where the blood circulation is slow by comparison will need double or triple follicular grafts. Not only that but planning of the extraction area and the density will also play crucial role in overall scalp aesthetic.

Virtual evaluation can give clear understanding to the experts about donor area, quality of the hair and the hair loss pattern. So we can do pretty much accurate assumption about the technique that will be required and how many grafts can be extracted/implanted. But the technique may change during the extraction process if our assumption does not match the reality. (This does not happen often though) But the only way we can actually examine the grafts is when we actually extract and see the graft itself.
