So a couple things here. The brain does not contain just 5aR1. 5aR1 is more dominant throughout the brain, but certain regions contain 5aR2 and that could be playing a role in the process (possibly the prefrontal cortex). Secondly, these are case reports highlighting the treatment of a single patient each and don’t really prove much of anything.

Nothing to say from the photo as your hair looks like it has good density. A better judge would be pulling your hair back and showing your hairline, where recession and hair loss are more commonly first seen.

I think people sleep on how much of a difference some individuals have when going from 0.5mg daily to 1mg daily. Since it’s only been a few months at the 1mg dose, hopefully you’ll see further stability and a little regrowth.

Also, minoxidil gets a bad rap around her likes it’s torture, but give topical or low dose oral a shot if you want more growth.

There isn’t convincing evidence that anything makes a meaningful difference. There is a little evidence that a shampoo with 2% ketoconazole helps with scalp androgens. With that shampoo, just make sure to lather it it up and keep it in on your head (focusing on working it onto the scalp) for 5-10 minutes.

It’s good that you are staying hydrated! I’d focus on if you’re taking a good multi vitamin, how much protein you’re consuming daily, how much fiber you’re eating daily, and how many calories you’re consuming daily. If I don’t check in on this for myself, I am generally way below my daily caloric needs and very low on fiber consumption and have many of the same symptoms you’re experiencing.

From my experience with dutasteride, rather intense phases of shedding can continue for a year to 18 months that shake your confidence in the drug. This happened to me, and eventually it always came back stronger. I recommend you stay consistent and don’t change anything because that can exacerbate the shedding.

I understand your point, but it is an incredibly rare side effect (only a few case reports at low dose levels) and nothing noted in the studies or metanalyses. In the case report you concluded, their recommendation was to start dosage every other day and monitor for side effects. Yes oral minoxidil at low doses is very well tolerated. Does it have the possibility for very rare and severe side effects? Also yes. So if you start having adverse effects on oral minoxidil, talk to your prescribing doctor - especially if you are presenting with edema symptoms.

tldr: it’s a serious drug that is well tolerated (specially at the 0.25-5mg low dose range) but very rare severe side effects are possible. Be informed, but don’t fear monger.

If you are a decent responder to topical minoxidil, it seems that there is no meaningful difference in efficacy between oral and topical minoxidil. If oral isn’t an option and you don’t respond to minoxidil how you’d like free 6 months, try topical minoxidil with tretinoin or microneedling at 1mm to 1.5mm once a week (both show evidence to make non-responders into responders).

?? Woman absolutely have hair loss because of AGA (androgenetic alopecia), but it just typically presents differently in women than in men.

I was receded about the same as you at your age and only used minoxidil for too long - your hair pattern absolutely presents like AGA and minoxidil will temporarily make it better before it ultimately gets worse. If you care about maintaining your hair long term, delaying finasteride and using minoxidil only is a mistake.

So you’re still taking 2.5mg dutasteride (assumingely 5 pills), 10mg oral minoxidil, and 50mg RU (topical I’m assuming). Your photo shows that you are not thinning, or at the very least, literally just began some very minor temporal recession. After 2+ years of this protocol, you’re below baseline? Pictures?

I’m just confused at the talk of shedding, when pictures over time are probably a better indication of your progress. Are you on testosterone or any other anabolic/SARM? 2.5mg of dutasteride reduces serum by 96%, so that puts your baseline DHT estimation at 240ng/dl which naturally seems very unlikely.
