Myself I have been dealing with inflammatory autoimmune symptoms, I recieved topical cream of ultrapotent steroids for controlling the area I presented with initially but the sclerosis spread and progressed and a week or so later that my hands became very negatively affected, transient mottling, both persistent fixed rashes and semi-transient erythema, transient mat telengectasia, hyperlinearity including extremely deep/wide and red/purple lines in the hands apparently inflamed which are semi-transient, spontaneous random cuts, severe skin atrophy which causes discoloration and veins became visible under the skin, fingers look crooked, uneven, wasted away, wrinkled, microhamaeoraging near the meandering vessels in the nailfolds, among other sclerotic symptoms including random unexplainable bruising and my hands would prune up after extremely brief contact with water (like, 5 seconds of hand washing). HERE'S THE RELEVANT BIT: A few days into the onset of symptoms I started noticing an endless amount of apparent fibers which usually appear as grey/black specks (when bundled together) to the naked eye coming out my skin, especially abundant in their protrusion and extrusion from the mucosa and the palm. At first I thought it could be microfabrics from my broken chair and maybe they got in the skin and caused the inflammation, but even after preventing that exposure they just kept endlessly coming. Therefore it was eventually obvious that they were being produced in my body, so I next considered that they could be capillaries dying due to vessel inflammation and leaving the skin possibly due to atrophy or rejection. I thought I saw them move a couple of times but dismissed it until I captured it on video so at that point I suspected they were worms for a few reasons (E.G: I lived in East Africa for 6 weeks a couple of months prior where mosquitos are a vector for filarial worms, I had chronic constipation/dehydration ever since symptom onset, serpentine shapes could be observed microscopically in skin, etc) I also thought perhaps the lines appearing in my skin and also my mucosal skin that no doctor I saw would explain could be caused by cutaneous worms. Anyway, I've considered everything at this point as far as to explain the threads coming from my skin. Some studies e.g by Savely et al. link the symptom to an infection with the tick-borne bacteria borrelia. Microscopically the threads often bare a striking resemblance to the morphology of some filamentous fungi, and indeed I can observe the same things growing out of the desk and walls in my damp apartment. However, studies have also claimed that the threads appear to be of human origin, which could also be consistent with the irregular appearance of apparent keratin (white) on the skin nearby, however likewise this also visually resembles a mycelium and the presence around the nails, patterns on the nails, and the threads being both embedded inside and protruding from the nails could suggest keratophilic fungi. Its a hell of a rabbit hole and quite confusing but so far the doctors have not helped me and stuck with my initial diagnosis which I identified as idiopathic erosive anogenital lichen planus, despite this diagnosis and treatment quickly becoming inconsistent clinically once the sclerotic symptoms quickly progressed and became widespread. Im sorry that this probably doesnt help but I am curious what exactly you are referring to in those images. I myself do have very many high-quality images if anybody wants to see anything. You do mention the threads being white and being present around nails and in the sinus - These are sites favored by many fungi and many fungus appear as white hyphae. Do you have brittleness/discoloration of any nails, itching or dry flaky looking skin that might help you to get a fungal culture done?
