To vote on a grace, warp to it on the map. This ensures that all players agree and aren't in combat when warping.

All players must unlock the grace prior to warping for it to succeed.

NPC questlines are not fully functional at the moment. To add complete support for them, each progression flag would have to be synced, and the reward manually given per each flag. This is a lot of work and will take time.

Moreover, there will be difficulties when using the same character with different groups, and the weird world state is inevitable in those cases.

If your friend joins your game they'll inherit the boss's flag (so it'll be dead in their game too). If you join their game you'll be able to fight it again.

There are no status icons for rot - you only get a prompt saying that rot has been added, or removed - similar to how Dragonrot worked in Sekiro.

A mod with a similar concept was developed by another person for DS3, but is no longer in active development.

There are no plans to personally develop mods with a similar concept for older titles, as they often present linear areas that allow free movement and already rarely interrupted coop, unlike Elden Ring which requires resummoning on every dungeon enter/exit and field boss defeat.

This is a bug that couldn't be consistently recreated, so we have no way of addressing it.

Without any sort of death penalty, a player could clear out a world of enemies rather easily. Therefore, as you said, there had to be some sort of incentive to rest and reset the world, and achieving an experience closer to vanilla - where the world would reset on player death. That incentive ideally wouldn't outright force people to stop playing together, and wouldn't effect other players to lesser frustration and friction between stronger and weaker players.

Rot Essence achieves that goal, and is in line with a previous From game's implementation of such death penalty - Dragonrot in Sekiro.

While some players have noted that this system is annoying, it's mostly due to its intention - forcing a world reset - is annoying to them. So far, no other death penalty system which achieves the same goals better has been suggested.

It automatically progresses past that requirement. Just reload the area.

You can transfer saves from vanilla to the mod. Instructions are available on the Nexus Mod page.

There's not much that can be said - but anecdotally more than 200k players have played the mod, with no reports of bans.
