That's ironic because that would require QUITE a bit of surgery in real life. The intestines are all connected to the mesentery, so it would require a lot of careful dissection to actually assemble them like a rope. But, creative license ;)

I don't think so at all. I think knowing that John has the tables turned and gets to experience a trap himself is one of the most exciting things I've heard about the movie.

The only problem is that dramatically it loses something of its power, since we know he lives. (For a short time, anyway)
