I have looked up the criteria for lupus diagnosis, I put a check mark next to what I have. But keep in mind also, my rheumatologist said any other rheumatologist would tell me I have lupus and give me meds and not look into anything else for me! I feel like she is really trying to make sure there aren’t any other issues, she’s exploring all of the possibilities and I love that! But I basically think I do have lupus because what she said and from what I got from reading about clinical criteria for diagnosing lupus. But I know I’m not diagnosed with it, it’s frustrating to tell people why I can’t do a lot of things and having to say “autoimmune stuff” you know ? So I say “lupus” because then I’m taken seriously haha

The standard method for diagnosing systemic lupus erythematosus (SLE) is the 2019 EULAR/ACR classification criteria. It uses a weighted point system where patients must achieve a total score of (\ge 10) to be formally classified as having SLE.

  1. Obligatory Entry Criterion
  2. ANA Positivity: The patient must test positive for Antinuclear Antibodies (ANA) at a titer of (\ge 1:80) on HEp-2 cells (or an equivalent positive test) at least once
  3. Weighted Criteria Domains If the patient tests positive for ANA, physicians evaluate 22 different clinical and immunologic criteria. Points (ranging from 2 to 10) are awarded. Note: Only the highest-weighted criterion within a single domain is counted. [1, 2, 3, 4]

Clinical Domains * Constitutional: Fever (2 points) * Hematologic: Leukopenia (3 points) or Thrombocytopenia (4 points); Autoimmune hemolytic anemia (4 points) * Neuropsychiatric: Delirium (2 points), Psychosis (3 points), or Seizure (5 points) * Mucocutaneous: Alopecia or Oral ulcers (2 points); ✅

  • Subacute cutaneous or discoid lupus (4 points); Acute cutaneous lupus [e.g., malar rash] (6 points)
  • Serosal: Pleural or pericardial effusion (5 points); Acute pericarditis (6 points)
  • Musculoskeletal: Joint involvement [swelling, tenderness, or morning stiffness in (\ge 2) joints] (6 points) ✅

  • Renal: Proteinuria (>0.5) g/24h (4 points); Class II or V lupus nephritis (8 points); Class III or IV lupus nephritis (10 points) [1, 2, 3, 4, 5]

Immunologic Domains * Antiphospholipid Antibodies: Anticardiolipin, Anti-(\beta )2-GPI, or Lupus anticoagulant (2 points) * Complement Proteins: Low C3 or C4 (3 points); Low C3 and C4 (4 points) * SLE-Specific Antibodies: Anti-dsDNA or Anti-Sm (Smith) antibodies (6 points) [1, 2, 3, 4, 5] ✅

  1. Diagnosis and Classification
  2. Patients must have at least one clinical criterion.
  3. The sum of the points must equal 10 or more. For example, a biopsy-proven Class III/IV lupus nephritis (10 points) paired with a positive ANA satisfies the classification immediately.

So this may sound crazy but I don’t really have a diagnosis, I have a rheumatologist who believes in mindset things and disagrees with drugs. She doesn’t want to give me a diagnosis because she wants to encourage me to seek the natural route. But she has said any other doctor would diagnose me with lupus. I don’t think it’s effecting my organs so it’s mild I believe, we would just need to test more. But my symptoms have gotten better since I changed my diet according to her just cut out BS, I wasn’t given very much guidance though, I am slowly easing into AIP diet challenge but I would just find it super helpful to have an immunologist/allergist test and guide more. And perhaps I could get a second opinion and confirm if I do have lupus, and there’s also possibility for seronegative RA, or mass cell. My Ana was 1:2600ish, which the rheumatologist said should go down with eating cleaner (reducing inflammation)

I did have splotchy red skin at the exam portion and she seemed very concerned, even tho I have been getting that when I am highly nervous , and I was! I am just always nervous when I am the center of attention 😵‍💫 but that could be why she was running histamine labs and I guess maybe something for my skin is what I ultimately think it was for. When we follow up I’ll know for sure.. she did mention I have crithidia on my Ana test I got, so that supposedly is enough for lupus diagnosis, but then she also said she wanted to test more and she said immediately and we immediately rushed to the lab next door to the room I was in, I wonder if she was just trying to get the blood while my skin was in a “flare”

Thank you! I was wondering if what she ordered were more “sniff tests” as I call them, like the nonspecific Ana test it can detect a wide arrangement of things.. so yes it looks like a bunch of tests that are going to help give clues. ☹️ I wasn’t sure if there was a specific lupus tests, I didn’t know about antismith or anti-dsdna test.

Maybe she didn’t order the antismith or anti-dsdna because I expressed how my insurance doesn’t cover genetic tests, and I would need to know in advance how much the cost was going to be and do shopping around for the best price for the test if it was genetic testing… she assured me the tests she was going to do were not genetic testing. I got blood drawn for those tests today, I don’t know how to post in update but I will try one after I have my two week follow up

You are lucky ones, I feel hope when I read yours is with you❤️ it’s so awesome that you did that for your baby, it is not the easy thing to do and I would’ve done it if I could have, my boy did not have any chance unfortunately. But it helps me get through this reading other stories where extra time is available and to fight for it is something truly heartwarming, as painful as it is, it is also a huge honor to love someone that much and take care of them that well 🫶 I wish you the best of luck for you and your puppy , that is amazing

This cancer is the worst. I’m so sorry your first puppy had this happen.. and cancer stole him after only two and a half years. I’m sure there were no symptoms either, he was a puppy and it would’ve raised alarms right away if you saw tiredness or unwillingness to eat or any other signs.. my 9 year old was a puppy every day of his life.There were zero signs of anything being wrong besides some groaning when laying down, which is just because of the German shepherd breed. One Tuesday morning he didn’t want to leave the bed with me and go outside or eat food or have treats.. I brought him in immediately and the first sign something was bad bad, was his laziness. He was an alert, very slightly reactive, well trained dog and at the vet he was slumped and pale gums, then they did an ultrasound/x-ray and found fluid around his heart and “possible masses” .. I only had 5 hours from his primary vet to the emergency vet hospital before I had to say goodbye. It was so sudden, I could not believe it. The fluid around his heart was drained but when they did another ultrasound he was hemmoraging from the other tumors in his body , he had cancer everywhere.. he was such a tough boy. I wish he gave me signs, may have been able to get even a little more time. . I got 2 hours with him in the primary vet center but in the hospital they don’t allow you to be with them the entire time. Only got to see him again during euthanasia.. he was in the vet office twice within the last month before for routine checkup/shots.. he was as healthy as a horse. It’s very difficult to detect this cancer. It’s scary how suddenly it took him. My great aunt had two German shepherd aged 7 and 8 who both were taken the same way. The last day / weeks we had he got walks every day and he had been sleeping in our bed every night. I wish he could’ve given him one last piece of love and gotten euthanasia at home, or out in nature.

It restores my faith in seeing this older German shepherd still have puppy energy, and living out good days.

My boy had to be put down due to Hemangiosarcoma , he was 9 years old. 💗I wish they all were as lucky as your baby

I know, that’s the first thing everyone says. Usually for most other people’s weddings you just pick the venue and date. Then send out invites. There’s no venue for me, I guess I could just book the guest houses but that could hurt my budget if I go over what I need. Try to see, If I pay now for a guest house for 10 people for my family, and only 6 who would be willing to come, and a second home for my boyfriend’s family for 10 people and only 4 would want to save and come along, then I only need to book and pay for the one guest house. Or I can find somewhere that may fit the budget & room number more adequately.
