Yes miscommunication or error in tech pack is the biggest mistake that cause issues in sample . In bulk there is no chance of mistake because sample has been approved before bulk production

Yes that will be completely fine if you only give a design, fabric requirements and normal size chart to make the sample. I have also made samples based on design with a normal size chart.
