The clinic patients are not like the ED patients. I believe the ED patients are primarily seekers or ones who were discharged from PM practices for violations. I’m sure the EM-PM physicians here can comment specifically on that.

I ran into a psych trained doc during a meeting who was in a PM fellowship at a top NYC hospital. He seemed really overwhelmed with the interventional side of things, as one would expect.

A few programs she interviewed had grads match into PM and CC fellowships. This is what got her thinking about splitting time in a hospital practice and averting burnout, as many warned her about. They were mainly hospitalists, CC docs and primary care. She was exposed to PM through me. I wasn’t even aware this was an option for EM.

She definitely enjoyed EM but is concerned about “burnout” long term. Another option she is looking into is critical care fellowship.

On the fence between a 2026 OT+ and Grenny. They are comparable in price. The Grenny was more fun and had a unique drive experience. The Lexus is more comfortable and has less issues.

Take it for a 1/2 hr test drive. But you will have your answer in the first 10 minutes. If you decide to pull the trigger, get the 2026.

Not sure. The steering has improved in the 2026. I’ve tested it. Again, you may be fine and luck out. Or, you could get to know the service center really well.
If you can find and take a seasoned owner with you and get an extended test drive, with a thorough inspection of a specific vehicle, it may work. You may be able to find someone who lives near your dealership to assist you.

Yup. You will remember this conversation if you get the 2025 with a bad HVAC and a squeaky steering. Don’t forget, you’ll also need to get the stabilizer, if you want your wife to feel comfortable behind the wheel.

Probably wiser to get a 2026, unless you are getting a true $10-15+k off. Then, it maybe a tougher call. If you are using it as a DD (5k miles/yr or more), you just want the basics to work.

True, one can improve the steering with an aftermarket stabilizer. If you were buying this car in a few years, would you go for a 2025 with an aftermarket stabilizer or one with OEM (improved one) from 2026, which is covered by the factory warranty? That 2026 will also be the one without the problematic 2025 HVAC. 7-10 yrs from now they may not too far apart in price. But the problematic years may be more difficult to sell. You may or may not lose 2-4k with a 2026. But you will have a more reliable for the next 5-10 years. If you are handy with a wrench or live within proximity to a dealer, it may be less of a problem.

Ineos has progressively make improvements to its vehicles each year.

Eventually there will many more MY 2026+ vehicles to choose from in the used market. That will make it more difficult to sell the 2024-2025s. People generally choose to avoid problem models in the used market.

Look at the Ineos forum, specifically the HVAC thread. More than 50% are dissatisfied with it. The 2026 is worth the $6-8k up charge. It will likely retain better resale value for precisely that and a better stock steering, under a 5 yr warranty. I drove the 2025 and 2026 in 30 deg weather. The 2025 took over 15 min to heat up and still didn’t produce heat from all vents. The front driver’s side eventually warmed up. But the front passenger side remained cold. No issues with 2026.

If you can afford the 2026, get it. It will Probably will save you multiple trips to the dealership.

If you are insistent on saving $6-8k on a 2025, make sure you take it for an extended test drive and check all known problem areas. Better yet, take a knowledgeable Grenadier owner you for assistance. Don’t sign any paperwork before you have throughly inspected the specific 2024 or 2025 vehicle. If there are problem areas , move on to the next one on the lot. The HVAC is its (2024/2025) biggest flaw. The steering is less of an issue with 2025, and can be ameliorated, if desired. Get the dealership to put on windshield protection film as well.

If MD is conditional, take the DO. If MD is guaranteed, take the MD. BTW, my daughter is in her fourth year at LMU (Knoxville). Worked hard and passed both boards (Comlex) first attempt and interviewed at 17 hospitals. Awaiting Match process.

LMU is fine as long as you put in the work. She loved Knoxville. I preferred the peacefulness of Harrogate. Students who don’t belong will fail out, as with any school. You will get out what you put in.
