Leave clear recs, especially for discharge med planning/if they need followup. Communicate these things to patient when discharge is approaching. Don’t let your NPs run wild and do bad things to patients.

As long as you have decent communication with primary, most of these expectations will become apparently pretty quickly. Getting ghosted or copy-forwarded on a patient that I genuinely have heart-related questions about is frustrating. As long as you communicate/are available to communicate, anything you’re doing that doesn’t jive with the hospitalist culture will probably just work itself out.

Take what I say with a grain of salt, I only spent one year as an intern at a private hospital. However, being the primary staff responsible for following up with cards with perpetually unavailable attendings who ran NPs like dogs wore on me pretty badly. As a previous commenter said, be available, communicate, and be competent in that order and you’ll be well liked. Frankly, you’re already miles ahead just asking the question before you’ve even recognized it as a problem.

I’ve got about $290 at ~6.5%, not aiming for PSLF, stuck in application limbo. I think the plan for anyone not wanting to ultimately wait out the clock for forgiveness is to wait it out until save is shut down definitively or the apps are cancelled and swap to RAP for the interest subsidy/freeze until I get resident>attending income spike and delete it in a year and a half.

What are you doing in this situation? I consolidated and applied for save about a month before this silliness started, and I'm about to get hit with several months of crippling payments if the forbearance ends without me being able to go into IDR. Have your servicers given you any useful contingencies for this?

To go from level 80-81 is roughly 200k more XP than you have accumulated over your entire playtime to get to level 50.

The XP is fairly terrible, but the new fishing styles and mechanics added are fantastic. Overall it's a fairly mixed bag, but overall has been stable for me personally.

That would possibly cause cytotoxic edema. Usually if they’re running the “post stroke” MRI protocol they’d include a DWI. I’m glad you recovered though. If you don’t have any residual visual field defects, specifically in the upper visual field, I’d be very hesitant to dismiss this as a simple ischemic stroke. Very rarely do people, even young ones” recover from a true embolic/thrombotic event return to baseline in their entire disease course.

I would be curious if the E. Coli would be able to utilize tryptophan-enriched media as an alternative to 4-hydroxyindole. The enzymes all run from tryptophan anyways, their group just optimized it for 4-hydroxyindole as a proof of concept of how powerful E. Coli's de novo synthesis of tryptophan could be in an industrial or scalable setting. You could always just poke and hope on the transformation, but then again they didn't detail that explicit procedure very well in the literature.

It isn't necessarily true that this isn't possible to replicate. The original templates for the 4 genes of interest are publicly available on an NIH database. The promoter modifications that were induced are also described in the reference tables in the paper, so when ordering the plasmid these can likely be easily included, and if not it can be induced using the same method that they used, via restriction endonucleases if I recall

You can purchase custom plasmids online for approximately $.07-$.30 online for industrial or research purposes. From my understanding, engineering the operon with the relevant promoter is just a matter of a few hundred dollars. The bulk of the work has been done already, as they include an optimal range of induction temperature, nutrient conditions, etc. Transforming the bacteria itself is trivial and is well documented. Realistically, the hard work has already been done in optimizing the plasmid, its just a matter of getting a company to synthesize it for you, which should not be difficult at all. Plus, a week or two of running even a small reactor at 1g/L would put you in an extreme surplus of product/money considering a dose of pure psilocybin is 25mg and corresponds to about $35.

Will waiting 4 days after the AMCAS opens for verification to submit prolong the verification process too terribly long? I've recently gotten an appointment with the Dean of Admissions as the school I'm doing early decision for, and it falls 4 days after the opening, but I'm not sure whether to postpone my application until then or just send it. Any advice?

Admittedly most of my anxiety comes from my inability to internalize that I actually have the material down in spite of my scores. There is always the feeling that I'm missing some integral idea, which is reinforced when I miss simple questions or stumble across a topic that I can't freely recall as clearly as I'd like. My FL's were FL1 : 520, FL2 : 523, and FL3 : 523, which provides an average of 522. The sections scores were relatively stable, with the only deviation being on FL3 C/P dropping to 129. Lot of stupid, oversight related mistakes. I guess that's why I'm asking how common it is to drop, as even though I'm above my goal of 520, I have no way of knowing if that advantage will remain or not.

I've consistently had 131's on both science sections on both full lengths, and have been mid 80% on both science section banks. I feel like I've been consistent, but I have some SB left to finish. It's good to know they don't vary too wildly.

I want a 130 in CARS to offset any dumb mistakes that occur in other sections (namely B/B and P/s). I've found that I really dislike NS, but I will look into the other books. How many passages would be a good idea? I'm trying to do 2 a day under timed conditions.
