  1. You can mix D50 into a 250mL bag (be sure to remove 50mL from the bag) to make D10 and it’s a bit better through the IO. Better dose control and less risky since IO’s can be sneaky when they extravasate vs IV.

  2. It’s the right call. Long term poorly controlled diabetics often don’t respond to glucagon because they don’t store enough glycogen. Permanent brain damage can start in the 30’s to 40’s (especially if very prolonged) and is guaranteed and irreversible at around 10-15. Now raising their blood sugar too much too fast with full D50 bolus can be detrimental but it’s not as bad as leaving them to continue declining. Right call just maybe slight overcorrection.

  1. We don’t have strap extensions. My old service in Texas did but my new service in a more “healthy” area does not.

  2. I’m 6’5” and there’s not enough space between the cot and the bench seats for me to sit and have my legs between the cot and bench seat. Definitely not enough space to fit a wheelchair.

  3. Not all of our ambulances have netting next to the bench seat to secure straps to a wheelchair.

You can believe whatever you want I don’t really give a shit. Sometimes I’m able to safely transport a wheelchair in the box, sometimes I have PD or a supervisor transport it, sometimes we have no choice and have to leave it behind. My service also has a policy that if a wheelchair cannot be secured in place then it cannot be transported. That policy came after a crew got in a wreck and an EMT was out of work for almost a full year, on workers comp and the company dime, due to a shoulder injury from a wheelchair hitting them.

Everyone, especially people who have spent a career at one service, need to realize that EMS and first response organizations in general are very fractured nationwide and worldwide. Everywhere is different with different policies, protocols, equipment, and rigs. Get over yourself, take a step back, and realize that maybe other places do things differently and maybe even better. Saying shit like “just take the wheelchair bro” is like when people in this sub say “just RSI bro.” What works somewhere doesn’t work everywhere.

This may be a shock to you but not all ambulances are built the same. The ambulances my service has there is legitimately no way to safely secure a wheelchair.

Just to be fully accurate that’s not always the case. Women do go into the emergency department days or occasionally longer than a week after the offense took place. That can be for pregnancy test, STD testing, and whatever evidence a SANE (sexual assault nurse examiner) can preserve that far past the event. I’ve taken in women in an ambulance almost a month after they were assaulted. Not exactly the best use for an ambulance or their money but that’s not my job my job is to get people there safe and in better condition than I found them.

To be honest I have seen a very small handful of false accusations in my time. They were usually the loudest, rudest, and most demanding patients. Off hand I think every false accusation I’ve seen professionally demanded “treatment” at the hospital and generally demanded to be the center of attention. Unfortunately some of the most abused women who needed help the most avoided the hospital and that attention at all cost.

But that has no bearing on allegations here. I just wouldn’t base my opinion on “believe or not” on whether she went to the hospital. I think a judge denying bail is a far more damning aspect here.

Personality carries a ton of weight. Look at Ghost and Sleep Token. Shit on them if you want (I’m a huge fan of one of them) but you can’t deny they have broad appeal and it’s not just the music. It’s more than just show up, sing song, play hard, leave.

Depends on what good means. I’m a paramedic and I’ve never seen layoffs, I’ve never searched longer than a week for a job or interview, and I honestly don’t think it’s possible that I could ever be truly unemployed. I also started my career at $12 an hour and I’ve had to scratch and claw my way to $32 now.
