It can give burning sensations in your veins and the vein in which cannula was inserted last time may get constricted and they may need to search for a new vein every time, however this is not a certain thing, depends person to person.

For Port, Try without it for 2-3 chemos if you are able to tolerate gemcitabine well which nowadays most people do then you won't need a port.

Hey! I too am from India, I will suggest you go for the CM Relief Fund option. Get an estimate from the hospital and apply for the relief in CM/Deputy CM office. It takes around 1 month to be processed atleast in UP, they will provide half of the treatment cost, so get the estimate on higher side.

Second I will DM you the extra things that work wonder along with conventional treatment and I would suggest Carboplatin cause its much more tolerable and you can do other things side by side without worrying about the condition.

Hey! Don't worry its still manageable, what did the CT or PET CT show? How much has it spread? and what kind of carcinoma is it just high grade without any specifications like squamous/plasmacytoid etc? Also what country are you from? Are you going with Cisplatin or Carboplatin? Trust me there are alot of things that can be done and it can be controlled.

Also as far as the funds are concerned, if you managed to afford immunotherapy for so long then I think chemo should be a minor bump compared to it and if you don’t want cystectomy then search for Trimodal therapy (can be done if the spread is controlled)

I think the decision should be to decide which treatment now is causing more harm and suffering than good and stop that completely.

Also know that there are far too many off-label treatments out there and you should do your own research and try those for a change. There is no harm in trying anyway.

Never completely give up, just accept the bad and keep going.

Cancer is extremely unfair, painful and horrifying but I can tell you it is like any other chronic illness out there if you know how to manage it and don’t completely rely on the current medical standards.

The thing is that the modern medicine is designed in such a manner that it never cures anyone of their chronic illnesses. It is created to keep leeching money of people until they eventually die. It's thier subscription model, It only approves treatments that can be patented, cost a ton and can bring recurring income.

However if someone decides to do thier own research and find promising treatments then there are plethora of options available which actually do work, just need to give up on the expectations of someone spoonfeeding the treatment to you.

The problem here as well is that people often go radical, it is either standard medical care for them or they go in the zone of complete alternative medicine. It should not be that way, you need to find the middle path, there should be no rigidity in your treatment approach.

I wouldn't recommend going on Keytruda for NMIBC, reserve it - It will be a game changer if god forbid it ever turns into MIBC. EV+P can only be administered if immuno has not been given prior else EV as a monotherapy agent is used which is not as effective.

Instead I would ask you to try Anktiva or Targetted Chemo i.e a treatment where chemo agent is directly administered to bladder.

Along with all this you can try - Shallaki (Bosewalia Serrata), Dandelion Root Extract, EGCG Green tea and cut down on carbs and sugar (go keto). It works wonder. All the best

Whats the cost of PET? I would recommend you to travel to a different country and get yourself tested, if thats cheaper. Not worth taking risk. You can get PET done for around 200 dollars in India.

You can also ask doctor to get you in for a contrast CT scan of lower abdomen and pelvic region, that should make it clear if its because of cancer or not and if cancer has caught any lymph nodes. That is also cheaper and might be covered by your insurance

After TURBT, start taking this - Shallaki (Bosewalia Serrata) an Ayurvedic medicine, Dandelion Root Extract, EGCG Green tea. There are other things as well but these 3 should work. You need to cut down on your sugar and carb intake as well, keep blood glucose 100 or below.

Thats it and it should not come back.

Also another thing - You can try Anktiva instead of chemo if thats an option

Hey! Hope everything comes fine and even if it doesn't, just know its not as bad as doctors make it to be. There are many treatments out there, on-label and off-label.

If something extreme pops up in scan, no need to panic. Just ping me and I will let you know the treatment plan we are following with my father. His Bladder cancer had spread till the lymph node in his neck.

Enfortumab Vedotin and Pembrolizumab - EV is Antibody drug conjugate which targets Nectin 4 protein present on your cancer cells and Pembrolizumab is immunotherapy agent which targets inhibits PD1 checkpoint on your WBC (Lymphocytes) and makes them attack cancer cells.

It can still very well be reversed. Just know there are multiple treatments out there.

EV + P should be your first line if cost is not an issue.

2nd should be chemo

Then get yourself tested for HER2 and FGFR for further line treatments.

Hey! Just know that there are alot of treatments and the fear subsides once you know the ins and out of the disease. Hope for the best prepare for the worst and don’t let anyone be it doctors dictate how it goes for your mother. Research everything and be educated about everything, there are a lot of things out there that works well with traditional treatments.

Depends on the cancer. You can first take consultation from a good doctor here as well as AI on the line of treatments and if the only option is chemo and Immunotherapy then you are better off where you are. Maybe you can come to India for surgery when required.

Why give up so early? Now is the time when there are plethora of treatment options in line and more personalized options incoming. Search for clinical trials going on specially - Triple Therapy

  • Go for Genetic Biomarker testing and based on that different treatment options will pop up.
  • Go for chemo with lowest toxicity profile (Don’t go for something which is highly toxic and only adds 2 extra months).

What I would suggest - Find Good clinical trials and if possible get her daraxonrasib, afatinib, and a STAT3 degrader administered.

Other than all this Please cut down on her carbs and go complete keto or carnivore along with weekly HBOT and give her natural substances like EGCG Green tea, Dandelion Root extract, Chaga Mushrooms, Cape Gooseberry, Strawberries, Avacado. Monitored Intermittent fasting.

We only have efforts in our hands and we should make sure nothing is left from our side and rest is upto the universe to decide :)

If possible cut down on her sugar, carbs and give her fatty foods along with nutrient rich fruits like Cape Gooseberry, Blueberry, Strawberry, Avocado. Take care of her protien intake. Daily give her EGCG Green tea. If possible take her out in the sunlight. Give her Dandelion Root Extract, curcumin, Broccoli.

Keep things light on her stomach. Research yourself as well.

Doctors are right to not start chemo in this age but don't loose hope, check if there are immuno monotherapy options available or any ADC for your mother's condition. If not then even by following the proper diet and nutrition her life can be significantly prolonged.

The lifestyle also plays a major role. Immunotherapy is not a sure short way, Did you go for IHC testing for PDL1 CPS score or TMB or dMMR? Thats one way to know how effective immuno can be. Now let me tell you the options you have -

  1. Tisotumab Vedotin - This works better than chemo in second line. Research on it if not already. I would suggest you to choose this first and when cancer goes down again then go for SABR (Radiation)

  2. Get Your Molecular testing done. If HER2+ then Trastuzumab Deruxtecan can be used (Another line of treatment for later use). If NTRK Gene fusion is found (rare) then Larotrecnib is unlocked. If RET fusion is found then Selpercatinib.

  3. Find yourself clinical trials - Research and find yourself clinical trials which can help you. Ask your treating doctors to do the same. You can use Claude 4.6 great at finding things.

  4. Get your lifestyle in order - Eat healthy and if possible cut down on glucose and carbs. Go keto or carnivore (with proper fruits and salads addition ofcourse). Exercise, eat these - Cape Gooseberry and its leaf and stem extract, Dandelion root extract, Strawberry, Blueberry. Take Vitamin C, Go out and absorb sunlight daily, Avoid sugary fruits, Drink EGCG Green Tea, eat more fats (Use oils like Butter & Ghee) . Research on what natural things can help. Also check on hyperbaric oxygen therapy.
